These signs may be compatible with a balance problem related to a vestibular disorder. Often middle ear problems are suspected in these cases. It is possible to have both the trigeminal neuralgia and vestibular disease but it is more likely that it is just the vestibular disease. What was originally thought of as trigeminal neuralgia was probably just early signs of the vestibular disease. Typically dogs with this diagnosis recover in 3-5 days and only need supportive care to get through the worst of it. I have however had a patient that took almost 2 weeks to become normal. Although old dog vestibular disease can recur it is less common then with other diseases that cause vestibular symptoms. It would also be unusual for a dog to have symptoms of this syndrome, be normal that quick (depends on severity but most dogs get better over 7 to 14 days) and then have symptoms again. The yelping is also odd as this could signal pain. If you are saying that your dog moving his head initiates the pain then he could have neck pain. I would recommend having your veterinarian examine Egor again with these new symptoms. With some disease processes it can be helpful to see the progression in order to know what is going on. If neck pain is detected your vet may recommend some x-rays of the spine and pain management. If neurologic dysfunction is detected on a neurologic exam he or she may recommand referral to a veterinary neurologist. Yes it can be the cause of vestibular syndrome and loss of balance. More likely this episode is due to a geriatric vestibular syndrome (usually self limiting) or otitis (inflammation of the ear canal) media. A check up and neurological and physical examinations are necessary to differentiate between these different clinical syndromes. Dental disease can cause severe infection in the mouth and throat which could potentially be inhaled into the chest as well as affecting the kidneys and heart. You should have him examined by a vet, he may require urgent treatment if he has developed a pneumonia

There is something called Old Dog Vestibular Disease, that causes dogs to suddenly appear to be totally unable to balance, be nauseous, spine in circles, or even be unable to stand. This happens for no particular reason, and usually improves over a few weeks with patience and supportive care (anti-nausea medication and fluid/nutritional support as needed). It is important to check for middle ear infections (which requires the otoscope at the vet clinic), general blood work and thyroid levels, and do an exam to make sure she doesn't have signs of other neurologic issues that would suggest a brain tumor. So, I'd advise keeping her safe tonight by blocking stairs, removing water boils, etc and taking her to the vet tomorrow to be checked out.
