Why are my dog's ears always infected despite Z/D food and washes?

Hi there. Recurring ear infections, or persistently itchy, irritated ears, are indeed a common sign of allergies in dogs. This can be due to environmental allergies, a food allergy, or both. Apoquel is usually effective at treating environmental allergy problems - so if it didn't help much, I agree that a food trial with a prescription hypoallergenic diet would be the next logical step. If she responds well to the prescription diet, you may be able to cautiously switch her back to an over-the-counter limited ingredient food for long-term maintenance.

Chronic ear and skin infections may have an allergic component as stated by your veterinarian. It may be fleas, but the likelihood is reduced with regular flea control. Even then though, in a sensitive dog, one flea bite may be enough to cause a serious allergic reaction. The steroid (Prednidale) and antibiotic (Rilexine) are temporary fixes, treating just the symptoms but not the underlying condition. There are also environmental allergies and food sensitivities that may responsible. The sheer number of potential allergens is what makes diagnosing allergies so frustrating. Discuss allergy testing, hydrolyzed diets and food elimination trials with your veterinarian. A consult with a veterinary dermatologist ma be indicated. You should also discuss antihistamine use, either seasonally or year round, with your veterinarian. Atopica and Apoquel are prescription medications that may help treat allergies and can be dispensed by your vet. Consider adding an essential fatty acid supplement, such as a fish oil, to Irish's diet to promote healthy skin and coat. The EFAs may also help some with the itching and inflammation. DermCaps or EicosaCaps are two recommended products.

I am sorry to hear of Princeton's ongoing war issues. If you are concerned about possible medication side effects I would recommend discussing this with your regular veterinarian. I would also discuss with them the possibility of food allergies. Chronic/recurrent ear infections like you are dealing with for Princeton can often be secondary for a food allergy, a diet change may be what is needed for him. When considering a new food remember that the protein source is typically the thing that causes the allergy-look at Princeton's current dry food to see what the protein source is. You will need to look for a food with a protein source he has not been exposed to (remember that the chicken you feed mixed with his food is a potential allergen as well). Your vet can discuss any particular brands or products they recommend, and be aware that a food trial for allergies can take 6-8 weeks before any effect is seen. Also talk to your vet about culturing the material in Princeton's ears, if you haven't already. Sometimes the yeast or bacteria that is present becomes resistant. A culture will show what type of organism is present, and what medication is going to be most effective. Your vet can then help you tailor a new treatment plan to try and resolve the issue. I also recommend routine ear cleaning at home using a cleaning solution recommended by your vet. Keeping the ears clean of debris can help reduce the number of infections. Itchy ears are most commonly a sign of allergies. Your dog may have food allergies, environmental allergies, or parasitic/bacterial skin infection. If he hasn't had a skin scraping to rule out parasites, or skin cytology/culture to look for bacterial or yeast infection, these things should be done. Some dogs actually have skin tumors or autoimmune skin disease that is causing the itching, and those dogs need to have fine needle aspirates or skin biopsies taken for diagnosis. Also, you should try a veterinary prescription allergy diet like Hills z/d for 8-10 weeks fed exclusively to treat potential food allergies. It's really important to feed ONLY the prescription diet during this time. Also, I have had great success with the oral pill Apoquel for some allergies as well, and this is a medication that is much safer to use long-term than steroids like prednisone. If this course of diagnosis and/or treatment is not possible with your current veterinarian, pursue a second opinion somewhere else. If all of these options are explored and your dog is still itchy, it's time to visit a veterinary dermatologist specialist.

First, if your vet has not performed an ear cytology to look under the microscope at the ear discharge, that would be a good idea to evaluate for yeast or bacterial overgrowth since there is a slight smell to the ear discharge. If there isn't any evidence of an infection or overgrowth that needs to be treated, then you might consider a product like Zymox. It is available as a cleaner or a medication. I usually recommend the medication Zymox Otic with 1% HC if the ears are itchy. Hi there, It would be helpful to know what Elsa what tests were performed, what was diagnosed, and what medication she was prescribed. It may be that her ear infection is truly secondary to underlying inflammation from a food allergy (cutaneous adverse food reaction), and she may need a change in diet, or an environmental allergy and in need of chronic allergy medication to reduce her itchiness. Some dogs also require long term regular ear cleanings and topical ointment applications to keep comfortable. I would recommend scheduling a recheck appointment with Elsa's veterinarian to get to the root of her ear infections. I hope this helps. I would try a prescription hypoallergenic diet. Your vet will be able to recommend the one they think will work best for her like science diet Z/D or purina HA. This will be your best shot to rule out any food allergies. I recommend to do a minimum of a 12 week food trial with NOTHING else except for the Z/D. Even her heartworm prevention, if you give it, needs to be hypoallergenic. Unfortunately, most of the oral ones have chicken or beef flavoring in them. For treats, you can take the canned version of whatever food you choose and put a small amount into an ice cube tray and freeze them. Then popped them out and put them in a bag in the freezer for treats. I would also consider putting her on both apoquel and cytopoint at the same time to help control itching. I would also talk to your vet about whether or not they think there is any yeast on her. Yeast can be very itching. There is a anti-fungal that can be given long term and be safe. It is called terbinafine. I had a dermatologist to give it once a day for 4 weeks then every Monday and Tuesday long term to keep the yeast under control in my really allergic patients. Your vet will be able to determine the best dose and whether or not she even needs this. Finally, if this doesn't help, I would strongly recommend to find a veterinary dermatologist in your area to see if they can do different types of treatment. Most dermatologists can do skin allergy testing and get her started on allergy injections if needed. I hope this helps!
