My 5-month Yorkie breathes like drowning after shower. Is it serious?

My guess is that during the bath he aspirated some water down into his lungs. I recommend you stop trying to give him water for now. You need to get him to a vet if he is lethargic (just laying around) and groaning. This could be a serious issue in a small breed dog that is so young. I would consider this an emergency. Unfortunately, without an exam it would be hard to say what is going on with Yusi. Issues such as allergies to something in the environment such as pollen, upper respiratory infections, issues with the nose, throat, or lungs, and other underlying health issues can all be possible causes. If this has happened more than once, is ongoing, or Yusi seems very ill or in distress, it is a good idea to schedule a veterinary checkup to rule out any health issues and get any symptoms treated.

That could be an early stage sign of tracheal collapse, or there may be a inflammation of the trachea or first airways (throat). Tracheal collapse is quite common in yorkies and is usually diagnosed by performing a radiographic evaluation of neck and chest. I would recommend a check up as if otherwise the signs are due to an inflammatory disease, anti-inflammatory and possibly antibiotics are required. Tanno needs to be examined by the veterinarian. A fatty mass is not expected to cause problems breathing. It could be coincidental. Problems breathing can be related to heart disease or lung disease.
