My dog's back leg is weak, she struggles to walk and loses balance. What's wrong?

Hello. I am sorry that Zara is hurting. Sometimes German Shepherds can have something called "Hip Dysplasia" and can have severe arthritis and abnormal hips and this can cause pain and trouble walking. I recommend you have her checked by your vet, who will likely perform x-rays to check her hips. Your vet can also help you get set up with joint supplements as well as pain management for Zara. In an older GSD losing ability to control hindlims, I would be concerned about degenerative diseases of the spinal cord (degenerative myelopathy-DM), spinal cord tumors, or less likely a fungal infection such as aspergillosis. Sadie may be having a bulging or ruptured disc issue in her back. She needs to be seen by a veterinarian immediately because the sooner this issue is addressed the better chance she has for a full recovery. Disc disease is very painful and can result in paralysis if not treated. Depending on the severity of the disease Sadie may just need prescription medication and rest or if the condition is more severe she may need surgery for a full recovery. Please remember there are no safe over the counter pain medications for dogs so you will need to have these prescribed by a veterinarian. Again, the sooner Sadie is treated the better chance for a full recovery so please take her to see a veterinarian right away. The two problems may be related or not. If they are, the underlying injury (soft tissue or bone tissue damage) is probably so severe to cause the loss of balance; in the the other case the loss of balance could be related to a dysfunction of the vestibular system (that part of the nervous system in charge of the balance). In both cases Sadie would require a physical examination to confirm these clinical suspicion and appropriate treatment. Radiographs may be needed. Oh dear! This is a very serious problem - I'm afraid she really has to see a vet right away. She may have a herniated disc in her back causing the paralysis, or some other neurologic problem affecting her spinal cord. She may be in pain, or she may have no feeling at all in her rear legs - it's impossible to say without an exam. She needs immediate treatment to have a reasonable chance of regaining function in her rear legs, I'm afraid there's nothing you can do at home that will help her. It sounds like what you are describing is weakness. Did your pup have a neurologic exam? What we look for is evidence of pain (although this can sometimes be inconsistent in pets), loss of sensation to the feet (does he fail to flip his foot back over when the top of his foot is placed down?), and presence or absence of reflexes. The results of these tests can help to determine if it's neurologic in origin and what portion of the spinal cord may be affected. Although, it can sometimes be hard to differentiate orthopedic from neurologic disease. Plain xrays of the spine are helpful to rule out any bony abnormalities, and sometimes we can see evidence of narrow disc spaces, but ultimately we cannot see the spinal cord, nerves leaving the spinal cord, or the soft tissues. If a neurologic problem is suspected an MRI is often needed. But, there are also some other conditions that can cause weakness that are metabolic in origin (has your guy had any senior lab work recently?) or related to loss of signaling to the muscles (for example a disease called myasthenia gravis) or related to a degenerative process such as degenerative myelopathy. If there is a component of pain detected on exam then anti-inflammatory pain medication may help. If you feel like things are getting worse it would be a good time to have a re-evaluation with your family vet. Another option as well, if a neurologic process is suspected, would be to have a consult with a veterinary neurologist. This sounds like severe arthritis of the hip, it could also be a neurological problem, possibly due to some sort of intoxication. I strongly advise taking her to the vet ASAP in order to get it treated.
