Can I give my dog antibiotics if he has high creatinine levels?

That depends. Ideally a culture and sensitivity is run on the urine to identify what bacteria is present and guide what antibiotic should be used. However, this is sometimes cost prohibitive. Cefpodoxime is a perfectly reasonable choice if we don't have a urine culture to go off of, or if we don't yet have those results. If Laddie is eating,is not vomiting or having diarrhea, and isn't in acute kidney failure due to the infection, oral antibiotics alone are probably just fine. However if she is vomiting, is not eating, or is acting very lethargic, she should be re-evaluated by a vet. It is wise to monitor the creatinine value in the blood. If it is elevated, often dogs are treated with IV fluids as well as antibiotics. The creatinine can be measured daily with a blood test. It is a high dose (22 mg/kg is average although there is a range because of capsule sizes) and in some cases we would dose reduce this drug with kidney failure (but if your pet has early changes this may not be necessary, is case dependent). Also, they can be used together. However, terbinafine should not be used in dogs with pre-existing kidney disease. Please, discuss with your vet if the treatment should be discontinued or not. It is a worry that the creatinine is going up and your vet may want to change or stop treatment for Bruno, just in case he has individual sensitivity to any of these medications.

Pets with renal disease need to eat a special dog food aimed at reducing the work load on the kidneys. Hill Science Diet and Royal Canin both make special formulas for this condition. Some of these pets need intravenous fluids for up to three days. Some of them need supportive care in the form of subcutaneous fluids, anti-acids and antivomiting medications. When elevated along with the BUN then the symptoms would be those of kidney disease - it differs for acute vs chronic disease. It is difficult to say if the elevated creatinine is due to the medications or not since you don't have a "before" creatinine. Meropenem is not known to cause renal side effects unless the dog already had a renal condition to start with. The known side effects of meropenem may include: nausea, vomiting, diarrhea, skin rashes, phlebitis, eosinophilia, transient elevation of liver function tests and seizures (esp. in patients with CNS dysfunction and renal failure).
