Is it unhealthy for an unspayed female cat to never have kittens?

It is possible for her to be having a false pregnancy. It is more common in dogs, but cats can have them as well. If you are not planning on breeding her it is best to have her spayed. False pregnancies can turn into a uterine infection (pyometra) which can be life threatening.

Since Baby is over 1 year, she can be neutered any time now. I would recommend that she is neutered for medical reason, first the high risk of developing mammary cancer which is malignant in cats and second uterus cancer and/or possible uterus infection. No, kittens have no effect on the fertility of humans. The only concern is that when a women is pregnant. Then she should not touch or clean the litter box due to the risk of toxoplasmosis from the feces of the cat. Otherwise, it's fine. Get an indoor cat, they are the best! I have four of my own :-)

No, that is not true. Spaying is a very common procedure, and it's typically safe. As with any surgery, there are risks (i.e. infection, bleeding), but dying from the surgery is rare. It won't affect Lily's physical abilities, although she will need a few weeks of rest to heal up from it. And she will be happy. I highly recommend getting it done.
