Elsa has got hairfall behind her ear and border of her ear has become really rough. She keeps on scratching it.

Fungal infections and or bacterial infections are possible. Skin testing like hair plugs and skin scrapes are needed to confirm this clinical suspicion. Bacterial colture for ear infection would be appropriate to identify resistant infections.
Unfortunately either systemic or local antibiotics treatment are needed. Osurnia (a type of ear drops) can be used as well for not cooperative dogs (administration of it is once weekly). Muzzle should be kept as last resource.
