My cat's diabetic insulin pen doesn't do half doses. What to do?

It sounds like Kosey's glucose is getting under control pretty well this time around as well which is great news. If you are ever worried the glucose is too low the safest thing to do is to not give any insulin so I think you did the right thing. It sounds like at this time the 5 units may be too much and you can start giving lower doses. To be sure this is the right thing for Kosey I would recommend having a glucose curve done at your vet (where multiple blood glucose readings are taken throughout the day). In general, try to make the injections as close to every 12 hours as you can. Most pets will still do well even if the injections are sometimes 13 hours apart.If it is going to be longer than 13 hours, talk with your vet about maybe adjusting the amount of insulin to give. You and your daughter should devise a treatment plan each week based upon your work patterns and write it down. Then keep the paper by the insulin and MARK down when the insulin was given (time). If the insulin and syringe is U40 then yes, 5 should be 5 units (it is important to use the same graded syringes and insulin as this would invalidate the graduations otherwise). Blood glucose should be checked resting and fasting as a meal or strenuous exercise will affect readings - a schedule of timings before meals/exercise should be arranged in order to compare readings. Changes the the dose rate given should be made only in consultation with the treating vet - we are not legally in a position to prescribe medication as the animal must be under the direct care and supervision of the prescribing vet in order to make these decisions

I'm always hesitant to disagree with another veterinarian, because she may well have valid reasons for choosing to do this - but, that being said, I have not personally ever heard of anyone starting a newly diagnosed diabetic cat on this high of a dose. Most internists recommend a starting dose of 1 unit twice daily for an average sized cat, or 1.5 - 2 units twice daily for a very large cat. If you're uncomfortable, there is nothing wrong with asking your veterinarian to explain her reasoning to help allay your concerns, or seeking a second opinion from another veterinarian in your area. If she is alert and more stable on her feet then I would not give any more supplements unless signs indicate that. Be sure to take her a glucose check before administering any more insulin though. I would recommend trying to feed her several small meals every 3-4 hours though to help maintain a higher blood glucose
