My cat's eye is barely open and he's lethargic. Is this serious?

Squinting typically means the eye is painful as does the lethargy. From your pictures, the underside of the eyelid appears swollen and his third eyelid is elevated. He could have an eye infection. It can happen alone or be part of an upper respiratory tract infection. He may start with other symptoms such as sneezing and a decreased appetite. Another possibility is a corneal ulcer or scratch across the surface of the eye. This is very painful. The doctor will examine him and may recommend a test to look for an ulcer. It could be a superficial eye infection, or even a flare up of an upper respiratory tract infection. You should indeed have skittles seen soon to make sure he does not have a deep corneal ulcer that requires aggressive medical intervention. Look for any signs of sneezing, nasal discharge, or bilateral ocular discharge as these may be more consistent with an upper respiratory tract infection. Based on what you are describing, I would recommend having Nino evaluated by a veterinarian as soon as possible. Extreme lethargy in a cat can be due to a variety of reasons (infection, pain, urinary obstruction, GI issues, etc) and the underlying cause for what you are seeing would need to be identified in order to get him feeling better.
