My cat's dermatitis is getting worse despite treatment. What now?

I would talk to your vet about doing a biopsy or a culture and sensitivity. The biopsy may be able to tell a different skin condition (underlying mange, fungal infection, etc) that may require different medications than just antibiotics and steroids. A culture and sensitivity may reveal a resistant bacteria that isn't responding to the current antibiotics. I hope this helps.

The wipes may be further irritating the area so I would discontinue the wipes and consider trying a topical spray such as Malacetic Conditioning spray (can be found online) and apply twice daily until resolved. This may also be a fungal infection such as ringworm so using the spray will help treat the potential fungal infection. If the area does not improve or it worsens, I would bring Marlow back in for a recheck examination with the previous veterinarian so that the most appropriate diagnostic and treatment recommendations can be made. This could also be some sort of infection perhaps. This requires further diagnostics which would be done based on an exam. Your veterinarian would be better able to proceed with the needed diagnostics to try to determine what may be wrong. I would highly recommend you have him reevaluated by his veterinarian since things have worsened again. They may be able to proceed with further diagnostics and treatments accordingly. Good luck!

However, I think a fungal infection like cryptococcus seems more likely and that would only respond to anti-fungal therapy. Another possibility is cancer such as squamous cell carcinoma. I recommend you consider having him reevaluated by your veterinarian for further assessment and treatment. May be a biopsy could be done at this time since it has not improved. Since your veterinarian was able to actually take a look at Sophia, I would believe their diagnosis. It could be that a lack of consistent response to therapy could be that the allergic component is not yet being addressed. Her staying in the litter box is likely due to discomfort which is why re-evaluation at this point is indicated. Your veterinarian could consider referral to a veterinary dermatologist for further recommendations. It really comes down to figuring out what is causing her allergies so that they can be treated or the specific allergen avoided. There are many things that can cause allergies though in most cases they can be broken down into 3 big categories: flea, food and environmental allergies. Allergies are usually worked up in a stepwise fashion starting with flea allergies as this is the easiest and cheapest to take care of. If not on a flea preventative, then it would be a good idea to get started on one. If already on one then it would be a good idea to change to something else (there are many new products out in the market). If not improved in 1-3 months then it is time to move on and check for food allergies by switching to a novel diet (new protein and new carbohydrate). If that does not seem to be the issue then you are left out having to check for environmental allergies which require blood and/or skin tests. Allergies can be very frustrating to deal with at first but so rewarding at the end when we are able to treat them effectively!

It does look like a skin infection but the origin (bacterial, fungal, yeast) I don't know just by looking at it. If it is bacterial, sometimes oral antibiotics are needed if topical therapy is not working. I don't know what cream you are using as I am not aware of a chlorhexidine cream, or how long you have been using it, so it's hard for me to comment much more on what you are already doing. I personally like these chlorhexidine wipes because they also contain an anti-fungal/anti-yeast component (climabazole) and a component to help repair the skin (phytosphingosine) so it is gentle. So, if she is licking, you will need to get an E-collar (cone) for her to wear until this is healed. So, if you want to try the medicated pads, I would do that. If there is still not resolution after a few weeks then she should go back to the vet. Skin maladies typically take quite a long time to respond completely to treatment so as long as it's not bothering her too much just continue on for a few more weeks.
