His SDMA is normal which is a more reliable marker for kidney function. He may have been mildly dehydrated which can cause creatinine to elevate. Your vet could run a urinalysis to check his urine specific gravity and then recheck his BUN and Creatinine in 3 months to monitor for any increases.

Creatinine of 8.8 is very high and indicates kidney disease/failure. This can be due to a back of urine from the bladder into the kidneys if he had a urinary blockage. Typically this is irreversible damage but only time will tell. Dehydration would not affect the creatinine to that degree. He should be put on anti-nausea medications which may help his appetite.

Remember, blood work is a snapshot of what is happening at the moment the blood was drawn. Twilight may have been a little more dehydrated at the last blood draw, & creatinine is a very sensitive marker of kidney function, more so than the BUN. So, be happy that the number is better today. Every lab has different reference ranges, so according to the lab that your cats blood was run in, the BUN and CREA are considered to be mildly elevated. However, unless your cat has a low specific gravity (concentrating ability of the kidneys), we have to assume that the BUN and CREA levels are not indicating any kind of kidney failure. In other words, if the kidneys are concentrating normally, and the pet is asymptomatic, then the BUN and CREA levels should simply be rechecked at a later date or another more sensitive test called the SDMA test should be done to clarify what is really going on with your pets kidneys. So based on the information you have provided we cannot say if Frodo has true kidney disease, but if his urine test was negative for bacteria then no infection is present. Ask your vet about Frodo's specific gravity and if that was normal or not. If it was normal, then go forward with the SDMA test to confirm that the kidneys are ok. There are many options, the one change which is proven to extend life expectancy and quality is to convert to a specialist kidney diet, preferably wet kidney food. The elevated creatinine on it's own does not indicate kidney disease. Remember, this blood was taken at one second in time, he may have recently eaten or been a bit dehydrated at the time which caused the CREA to be a bit elevated. Creatinine level of 2.4mg/dL means mild to moderate elevation. Creatinine level goes up with kidney diseases but it also can go up after a large, meaty meal or excessive excercise. Some dogs with a lot of muscle mass have naturally elevated creatinine, not related to kidney problems. SDMA test will confirm (or rule out) kidney disease in Charlie's case. Please, note that SDMA is not a better or more specific test that creatinine, it is just another blood test to check kidneys funcion. In the best case scenario, Charlie had elevation in creatinine because of aforementioned physiological conditions and his kidneys are ok. In the worse case scenerio, he has early stage of kidney disease and will be put on a special diet.
