Unfortunately kidney disease in an older cat is generally a progressive disease. The treatments you are giving Mesi now are probably helping her feel better but the disease process is still continuing. Her kidneys will continue to shut down over time. I would recommend continuing the treatments for now if it is helping her feel better, eat, etc. You can also try to space out the treatments (like giving them every other day) instead of daily and see if that is still beneficial for Mesi. This is from the manufacturer's website with regards to storing Eprex: Store at 2°C to 8°C. Do not freeze or shake. This temperature range should be closely maintained until administration to the patient. Store in original package in order to protect from light. When the product is about to be used, it may be removed from the refrigerator and stored at room temperature (below 25°C) for a maximum single period of seven days. So, if you keep it refrigerated at 2-8C even after opened, it should be OK to use. You could always call the manufacturer (phone number should be on the packaging) to get better council.

So you are doing the right thing by feeding Smokey this diet. You can give him the vaccine, it will not effect his kidneys. Water intake should never be less. It's important for him to drink plenty of water so he does not dehydrate. I recommend getting a pet water fountain for him. It is safe unless Mel is allergic to it (quite rare). The efficacy of it is a source of discussion and honestly I have never seen great benefits from its administration. The food you are giving is perfect and is part of the treatment for chronic renal failure in cats. K/D prescription diet (Hill's) is a good quality food as well.

They are formulated to reduce the amount of work required for the kidneys by reducing the amount of waste products produced from food as well as containing tightly balanced mineral, vitamin, omega fatty acid and protein levels and work best in wet forms or where good water intake can be encouraged. The larger proportion of the diet the better, any additional food sources should be highly digestible and suitable for pets in kidney failure, blood tests would be recommended 3-4 weeks after any diet changes to ensure no negative impacts develop

Wow, 23 is very old. You must take very good care of him. I am sorry to hear about his diagnosis. Renal disease as you probably already know is a chronic condition. Dietary change can be a major factor in extending the length and quality of cat's live who have it. However, in a 23 year old cat I would usually recommend just feeding whatever makes him happy. Nutritional intake in general is very important right now, so having to fight with him to get him to eat will end up being quite stressful and maybe counterproductive. If you are dedicated to a kidney friendly diet and the commercial stuff isn't working, you can certainly start thinking about home made diets. This is a link to a website that will guide you into making diets that are good for patients with kidney disease. I believe it is free for the first 14 days.
