My 13-year-old cat's bloodwork: Is it kidney disease or dehydration?

Every lab has different reference ranges, so according to the lab that your cats blood was run in, the BUN and CREA are considered to be mildly elevated. However, unless your cat has a low specific gravity (concentrating ability of the kidneys), we have to assume that the BUN and CREA levels are not indicating any kind of kidney failure. In other words, if the kidneys are concentrating normally, and the pet is asymptomatic, then the BUN and CREA levels should simply be rechecked at a later date or another more sensitive test called the SDMA test should be done to clarify what is really going on with your pets kidneys. So based on the information you have provided we cannot say if Frodo has true kidney disease, but if his urine test was negative for bacteria then no infection is present. Ask your vet about Frodo's specific gravity and if that was normal or not. If it was normal, then go forward with the SDMA test to confirm that the kidneys are ok. Hi there! After looking at the lab results, I see that all of these are extremely minor changes - they usually don't mean anything at all, in a dog that's clinically healthy and acting normal. The one possible exception would be the elevated BUN, as this can sometimes be a sign of early kidney disease - but it can also be caused by mild dehydration (common in anxious dogs who are panting at the vet's office!), a high-protein diet, or other things that don't have any clinical significance. If you wanted to be sure that his kidneys are fine, your veterinarian could do a urinalysis to make sure that his urine is appropriately concentrated and doesn't contain protein or any other abnormalities. The other values will often be slightly variable from one patient to another, and should not be any cause for concern.

Hello. It is possible that the new food is helping the creatinine levels, but regardless of the reason, this is good news. Remember, blood work is a snapshot of what is happening at the moment the blood was drawn. Twilight may have been a little more dehydrated at the last blood draw, & creatinine is a very sensitive marker of kidney function, more so than the BUN. So, be happy that the number is better today. His SDMA is normal which is a more reliable marker for kidney function. He may have been mildly dehydrated which can cause creatinine to elevate. Your vet could run a urinalysis to check his urine specific gravity and then recheck his BUN and Creatinine in 3 months to monitor for any increases.

Kudos to you for being such a diligent owner to your senior kitty! SDMA is technically a more sensitive measure of early kidney disease so the fact that it is still normal is excellent news. You’re right that the BUN and creatinine are borderline high, however mild dehydration can do this too. Therefore, if I see values like that in conjunction with highly concentrated urine and a normal SDMA, I’m not going to do anything except to continue monitoring. If the urine is dilute, there is protein in the urine, etc. then I investigate further. All in all, I agree that there is not much cause for concern but I’d recheck in 3-6 months. Thank you for sending over the copies of Gonzie's blood work! Looking at the provided results, I see that his creatinine is a tiny bit elevated. This value increases with kidney damage or dehydration. I'd certainly recommend talking to his vet about rechecking his kidney function in a month or two, as well as running a urinalysis to see how well his kidneys are working to concentration his urine. The monocyte increase is extremely mild. I don't worry when this type of white blood cell increases a tiny bit while other white blood cell lines (especially the neutrophils and lymphocytes) are normal. Stress alone can trigger this minor elevation. The RDW helps to give the proportions of the red blood cells' size and volume. Again, this minor elevation does not seem to be significant. A 50% hematocrit is slightly elevated, which means that Gonzie has some extra red blood cells. I would much rather see too many red blood cells than too few! His blood volume could also be more concentrated if he's a bit dehydrated. Platelets can be challenging to read on CBCs since they can clump together, so the machine may read a clump of several platelets as just one single cell. In these cases, it's always useful to manually examine a blood smear under the microscope to determine if adequate numbers of platelets are present. Nothing on the provided lab work makes me terribly concerned at this moment, but I do think it's worth tracking his kidney values and confirming an accurate platelet count. Slightly elevated BUN can indicate very early renal disease or dehydration. SDMA is a new lab value and indicates renal disease, though it is not consistently able to diagnose renal disease. I would recommend that you talk to your vet about concerns for the kidneys and to discuss if you need to change the diet or start fluid therapy. Decreased muscle mass in the back legs is common in older pets. It is from decreased use of the limbs. Encourage Beau to get gentle exercise through play so that he can maintain muscle mass.
