My cat is bloated, peeing a lot, and won't eat. What could be wrong?

Thank you for using Petco Pet Education Center, formerly Petcoach! From the X-rays, I can see that Mara's abdomen appears to be filled with fluid. It is obscuring his liver and intestines from being seen and there may even be a small amount in his lungs. Unfortunately it is impossible to tell what the fluid is from the x-ray alone. His blood work shows some abnormalities with his electrolytes, which may be if he is not eating or from fluid in the abdomen. Mara also has a high white blood cell count, these are the cells that are responsible for fighting off infections and increase if there is one present. Your vet can run some additional diagnostics, and perform a tap and remove the fluid as well as a blood test to determine if Mara is infected with FIP, a mutated form of a common virus that cats are exposed to. Possible causes are: Intestinal parasites, urinary obstructions (unable to pee, this is life threatening), fluid in his belly (from feline infectious peritonitis), ate something that is stuck in his stomach or intestines, feline leukemia, a fever from an infection. His symptoms are too vague and can mean many things. In order to treat him he needs a diagnosis from a Vet. I hope this helps and I wish you both the best!

Fluids in the abdomen can be caused by a lot of things and it usually makes the xray non diagnostical, at this age FIP can be one of the causes but it could also be a severe gastrointestinal infection or a liver problem. The next stage will be a specialist ultrasound, fluids might need to be drained first.

This sounds like he has a serious medical problem. He could be in renal failure, have an infectious disease like feline leukemia or aids, or this could be a sign of cancer. Regardless, I recommend a veterinary exam. They may do lab tests or X-rays to diagnose his medical condition and then they can start treatment. With the description and blood results, I worry about a condition known as hepatic lipidosis (fatty liver disease) or triaditis (inflammation of the pancreas, liver and small intestines). For fatty liver disease, a feeding tube is generally recommended to help bring cats out of this condition as nutritional support is key and extremely important in these cases. I would recommend an abdominal ultrasound as the next diagnostic step to help better determine what the cause could be. If Kitty is still not wanting to eat, you need to consider hospitalization and feeding tube placement to help her get through this. I hope this helps guide you in the right direction. I'm so sorry to hear that you and Sam are going through this scary situation. With normal blood work and the normal ultrasound, I would be concerned about the disease called FIP (Feline Infectious Peritonitis) which is an uncommon response to a common virus in which the abdomen fills up with fluid and cats don't tend to recover from it. I would also continue to be concerned about cancer even though you're not seeing any signs in the fluid or on the ultrasound. A tumor has to be at least a centimeter in diameter before it can be seen an ultrasound, so it's not unusual for something to just be too small. Unfortunately, without a diagnosis, it's incredibly difficult to treat. And even with a diagnosis, at his age, it's mostly bad things that it could be. At this point, your vet is probably focusing on palliative care to keep Sam comfortable by removing the fluid, keeping him well hydrated, making sure he is comfortable. I would recommend talking to them about his prognosis and your goals for him.

BUN, Amylase, magnesium and CPK all can become altered with renal impairment. The weight loss may be due to muscle wastage due to chronic renal disease or something else may be going on. I would consider having Tigger's urine specific gravity checked and also send out an SDMA test to see if the kidney's are the main problem. At 14.5 years of age, renal disease would be high on my list of causes of these symptoms. Other problems such as cancer and inflammatory diseases may be the cause of the symptoms as well. I would recommend having your vet pursue additional testing to get more of a definitive answer on whether or not the kidneys are impaired and if not, coming up with further recommendations on diagnostics and treatment.
