Your vet may recommend additional testing such as bloodwork or x-rays to check for any deficiencies or obstructions that could cause the behavior. If he checks out healthy, then looking to stress as a cause is a good idea, especially if this has happened twice after the same event. It may be that he would be better with a cage-top play area instead of free roam of the house, or having his cage moved to a lesser trafficked area of the house until he is more comfortable. Helping him get used to "scary" things by offering treats, interacting, and spending time together can also help to reduce stress in the long-term.

If he continues to act abnormally, I would be concerned about an injury and recommend he visit his vet. A loud scream definitely doesn't mean he hurt himself, but sustained lack of interest in play would make me very concerned. I hope he continues to return to normal.
