Try feeding Hikari Micro Pellets. The smaller size may make eating easier for him. You can also offer live blackworms as well as bloodworms. Confirm adequate water quality, proper water chemistry and adequate temperature. Bettas do best between 78F and 80F. If he is in a bowl or tank with no filter, weekly (or more frequent) water changes may be necessary.

Check the water chemistry first. Use a home test kit or submit a water sample for testing. Most pet stores will do this for free or for a small fee. Aim for an absence (0 ppm) of ammonia and nitrites with a nitrate level of 20 ppm or less. Increased levels of any of these can be toxic to fish. If indicated, perform a partial water change to stabilize chemistry or improve water quality. Use only conditioned water that is temperature matched to prevent further stress or shock. Bettas are tropical fish and do best between 78 F and 80 F. Low temps can affect appetite and activity in fish. Use a small, submersible heater if necessary. Fast him for 2-3 days, then feed small bits of cooked and shelled green peas. If he is constipated or has swim bladder issues, this should help. Feed peas until he is swimming and defecating normally. You can soak peas and pellets in some garlic juice to stimulate his appetite and provide an immune system boost.

Hi, and thanks for your question. This is a quite concerning but generic presentation and a picture of the pinkish area would be essential in this case to give you a more specific advice. Generally speaking, I would recommend to submit a water sample for quality testing. Most pet stores will do this for free or a nominal fee. Clinical examination should always include a water quality check, even if you have done so already. Water testing should be performed with a quality test kit, measuring NH4, NO2 and NO3. High ammonia levels, PH imbalances may be responsible for these signs. Oxygen concentration and quality of the filters should be determined as well. If water quality is good and there are no major imbalances, your fish should get examined to look for any sign of bacterial/fungal infection and eventually water should be treated. A betta's life expectancy is 3-5 years. He may be at the end of his span. Check the water quality and water temperature. Either can cause stress and illness if not set properly. Water temp should be between 78 F and 80 F. A drop below 74 F can be dangerous. Use a home test kit to check for high levels of nitrites, nitrates and ammonia. Correct levels through water change with treated/conditioned water. Make sure he is able to reach the surface to breathe. You can drop the water level if necessary.
