My old dog is sick and in pain. When is it time to say goodbye?

I am so sorry about Nubby. It definitely sounds like it is time for an exam/consult re: humane euthanasia with a veterinarian you are comfortable with. Yelping when moved, panting and shaking when at rest are likely signs that Nubby is in pain. Incontinence and inability to use her back legs are definite factors that diminish quality of life. Harley needs to have a wellness visit with your veterinarian who, depending on the physical exam, may want to take xrays of her heart and lungs, and do blood work to evaluate her liver/kidney function. Harley could have a congenital heart or liver issue, or something going on with her upper respiratory tract. All of these symptoms are to be expected at old age, it depends how severe they are and if medical treatment is helping or not, if the symptoms are severe and meds are not helping then it might be time to let Noosa go.

It does sound like Harley is feeling sick, and this lethargy could be caused by a systemic illness, infection, inflammation, or injury. The runny nose could be due to a respiratory infection or allergies, while her looking at her behind could be due to impacted anal sacs, pain, or intestinal worms. I do think it would be a good idea to have her seen by a vet tomorrow, and I don't feel you need to rush to the the vet tonight if she is eating and drinking. But definitely have her seen by a vet in the morning for a health check if she is still acting this way tomorrow.

I would recommend making an appointment to see your veterinarian as soon as possible. If Harley is favoring that leg and in pain, your veterinarian will be able to perform a full examination and suggest a further course of action, this may include imaging (ie X-rays) to assess that leg, and/or provision of appropriate anti-inflammatorys and pain relief as needed to make Harley more comfortable. Do ensure that you tell your veterinarian which products you have given to Harley so far so that they can ensure there are no interactions between products.
