My senior dog has mammary cancer. Can she have surgery at her age?

If it is possible, I would recommend having the x-rays done without sedation and depending on how the x-rays look and how her blood work looks, consider removal of the mass under anesthesia. She is not too old for anesthesia but the risk of anesthetic complications rises with age. As long as her heart, lungs and blood work are normal, she should be fine under anesthesia. The next step before considering surgery would be proceeding with a CT scan to check on how invasive the tumor appears to be. Then the next treatment option can be determined. A FNA can be performed at the time to determine the tumor type. If not too invasive surgery may be possible. If too invasive, and this is a thyroid carcinoma (the most common tumor in that region), radiation therapy or chemotherapy may be next best options. The prognosis with these tumors depends on the type which is why that needs to be tested. Also, it depends on how invasive it is and if it can be resected. Assuming Kara is in good health otherwise - which should be established with blood work and x-rays of the chest, to ensure that the tumor hasn't metastasized - then I would recommend proceeding with surgery. Mammary tumors can be quite invasive and aggressive, but depending on the grade surgery can be curative. Just because a dog is old, it doesn't necessarily mean they can't have anesthesia - you just have to take precautions, and find a vet who is capable of taking special care. I would also get a definitive diagnosis as to what the mass is before making any decisions, There are lots of benign lumps that grow all over dogs' bodies, and just because this one is in the mammary area, it isn't necessarily an aggressive cancer. A fine needle aspiration biopsy can usually tell you what these are.

While it is impossible to tell exactly what type of tumor it is and give a prognosis following removal, it is likely that removing the tumor will make him more comfortable. Depending on the approach, the surgical site may be rather large and require some care and pain management at home. Eighteen years is quite old for a dog, so I understand why you may be hesitant to go through any procedures that may decrease her quality of life. At this age it's uncertain how much longer she will live (with or without the tumor) and it seems like she may have other problems,or possible spread of the tumor that would not give a favorable prognosis for recovering from surgery. I would definitely want to solve the problem of not eating before recommending surgery for the tumor. It was helpful to see the notes and lab work, but I would need to further evaluate her and discuss this more in order to give you some more helpful advice. I think it would be worthwhile to discuss this with your veterinarian again and ask what they would do if it were their dog. This may help you with your decision. Sometimes during surgery it is difficult to remove all the cancer cells. Cells are microscopic and unless very wide margins were taken at surgery some cells may have been left behind. It is not necessarily the vet's fault. Cancer is difficult. The same thing happens in humans. Age is not a disease, if she is relatively in good health there is no reason for her to be unable to have surgery.

It sounds as if your vet is concerned that your dog has a malignant tumor, most likely squamous cell carcinoma. If there is suspicion about the malignancy of a tumor the best course of action is to have it removed. I can understand your concerns about Dahlia undergoing surgery but it would be safer and easier to remove it now, while it is smaller, then it would be if it was left to continue to grow.
