Senior dog with enlarged liver and tumors: when is euthanasia best?

If Bailey has not had a recent exam with a veterinarian, I recommend that be done soon. Both heart disease and liver disease can cause fluid to build up in the abdomen and make pets feel puny and puts pressure on internal organs. Several different diseases can cause enlargement of the liver, including hepatitis, cancer, and Cushing's Disease. Depending on the cause, some of these can be successfully treated. It can be very difficult to decide when it is time for a pet to leave this world. The "right" time will be different and completely OK with each individual pet and family. I recommend that you discuss Bailey's quality of life with your vet. You may also look for a vet that specializes in end-of-life care or pet hospice (sometimes called pawspice). Getting down to the cause of the liver failure is important for treatment. I would be concerned about the lumps and irregular breathing pattern. Chest x-rays would be helpful to determine if there is an abnormality with the heart or lungs or enlarged lymph nodes within the chest. The lumps could indicate cancer, either primary or secondary to another location. The liver is a common site of cancer or metastatic cancer. I would recommend talking to your vet about a liver biopsy or biopsying the masses to determine if they are cancerous. Antibiotics and steroids can help with infections of the liver or an inflamed liver, but if there was poor response then these are unlikely the causes of the elevated liver values. Denamarin is a supplement that contains antioxidants to protect the liver; it does not treat liver failure.

Liver tumors may or may not be truly "cancerous", i.e. Cause metastasis to other parts of the body and severe disease. Dogs can have benign liver tumors that don't metastasize, however they can get very large and impact liver function. There's really no way to know which type of tumor - benign or malignant - your dog has without a biopsy of the tumor. Ultimately the increase in water consumption is concerning, as polydipsia (increased water intake) can be a sign of severe liver disease. Some blood work would help to understand whether there is significant disease, as well as possibly an ultrasound exam of the liver. There may be medications that she could benefit from that would improve how she feels, however ultimately it's likely that addressing the tumor surgically - if that's even possibly - is the only way to definitively improve the situation.

Poor guy. It is tough when you are in these situations where you are deciding on continued care (which isn’t 100% guarantees to work) or Humane euthanasia. Unfortunately we don’t have enough information about your cats particular case to help you decide things here. Perhaps a consultation would be best to comb through all of the details such as lab work findings (even seeing the normals is quite helpful), medications tried so far, and his physical exam findings would also be helpful. But generally speaking if your pet is ill from liver disease, which can be any condition such as hepatitis, hepatic lipidosis (carries a poorer prognosis unless aggressive treatment is implemented including a feeding tube in most situations), liver cancer or even gallbladder disease can cause elevated liver enzymes, then it’s best fo an abdominal ultrasound to determine what specific liver condition is present. The prognosis of your pet should be determined by your vet and then carefully considered when determining if continued care should be done or if Humane euthanasia is the better option. Hello. Without the bloodwork, I can not address her overall condition to say whether surgery is appropriate or not. I would highly recommend that x-rays of her lungs be done to see if there is any evidence that the mass on her liver has metastasized to her lungs. If this has happened, then I would not put her through surgery. An ultrasound of her abdomen would also be very useful in looking at the liver, gall bladder & kidneys, & would give much more information about what is going on. In the meantime, I would start her on insulin to try & regulate the elevated blood sugar. Talk to your vet to see what the next best steps for Layla are. The only thing I would recommend doing is seeing your veterinarian. If Baby has an enlarged right heart and an enlarged liver it's possible that she is in right sided congestive heart failure and that this is causing her decrease in appetite. They may also be unrelated. A decreased appetite should be taken seriously in an older dog. The vet might recommend checking blood work or x-rays to determine what the cause may be, and from there they may be able to recommend treatment as well.

I'm sorry your dog isn't feeling well. Regarding your comment about leaving kibble in his bowl without eating it, yet still eating 2 cans of food a day and some treats--it sounds like he is getting enough food and appetite is not a problem. When you say he is "pooping everything out," is he having diarrhea? Or does it seem that he is losing weight even though he is eating a lot? Or is he pooping very frequently? To review the list of problems/symptoms that you have given: *losing weight even though he is eating a lot (if this is what you mean by "pooping everything out") *uncomfortable/moaning *enlarged liver *blood test negative for Cushings disease Considering that he is 13, given the problem list above one of the biggest things I would worry about is cancer of some kind, and arthritis may be contributing to his discomfort. Unfortunately there is not any ONE test for cancer, so its not something your vet can just submit a blood sample and get an answer for. If you are wanting to KNOW what her diagnosis is, your vet will need to run full lab work (a full chemistry profile, a complete blood count, and a urinalysis), take X-rays of his chest, and probably perform an abdominal ultrasound (sonogram of the belly) as well. If an abnormality is discovered on X-rays or ultrasound, a biopsy may needed. Regardless of the cause, and even if you don't have funds to do a lot of diagnostics, you can focus on making him comfortable, with the understanding that his time may be limited. Ask your vet about pain medications. Don't worry about him not eating kibble--if he prefers canned food that is fine. I'm sorry I cannot give you black and white answers as to what is wrong.
