Can you see sarcoptic mange mites on your dog's skin with your eyes?

Demodicosis and scabies are caused by 2 different mites, Demodex and Sarcpotes, respectively. The only way to diagnose and differentiate between these mites is by a veterinarian performing a skin scraping. This technique essentially scrapes away the top few layers of skin cells and bit of blood just under the outermost cells so that the sample can be examined microscopically. The mites are diagnosed during this microscopic exam and can easily be distinguished. There are two different prescription based medications for each mite, but must be given under close veterinary supervision. The absolute best way to know is if you see resolution of skin lesions and also when there are no mites detected on a skin scrape/cytology. Mites can still be present on the skin, even when the skin looks great on the outside. This will cause people to stop treating presumptively, and this results in treatment failure as the mites will come back quickly. I would be very suspicious of mange just looking at this. However to truly diagnose Demodex requires a deep skin scrape as this mite lives very deep in the hair follicles. It could also be due to ringworm as well as a bacterial skin infection.

A lot of times it depends on the type of mite. Some larger mites like Cheyletiella can look like white flakes. Other mites are microscopic and bury around the hair follicles. Sarcoptes mites usually cause extreme itchiness and hair loss especially around the ears. Demodex mites tend to not be itchy but can cause patchy hair loss and scabbing all over the body.

Since there is a smell, severe pruritus (itch), and hair loss this is very suspicious for sarcoptic mange, especially since both your dogs are affected. You will need to take them to your vet for a skin scrape to verify that it is indeed mange. Mange is relatively easy to treat with an injection of an avermectin. Sometimes a second injection is needed. Unfortunately sarcoptic mange is contagious and if you have other pets (dogs, cats) they should all be treated. You will also need to watch out for itchy skin conditions as humans can also be affected.

Without seeing it in person, I can't say if it's mange or ringworm for sure. Ringworm is typically a circular lesion that is red in color. Mange can be demodex or sarcoptic, but again without seeing him in person, I don't know which type it is. At the vet, they will likely use a Wood's lamp to see if the spot is ringworm, as infected hairs can fluoresce under the ultraviolet light. Not all cases of ringworm will do this, so a fungal culture may be needed. To diagnose mange, the vet takes skin scrapings, and looks at the sample under a microscope. The demodex mite is typically easily seen with a skin scraping, while it can take several skin scrapings to find the sarcoptic mite. Once the cause is found, the vet can discuss treatment with you.
