Why is my puppy passing a white worm after heartworm medicine?

Yes, it's very common to see the worms after giving the deworm medication. This type of deworm medication works by inhibiting the nerves in the worm. While this usually does kill the worm, they may be somewhat alive still when they are passed in the stool. The fact that the worm came out indicates that it is at-least debilitated. There are no worms in the medication. This could have been a cutting line or from the manufacturing process and I doubt you have to worry. There is a possibility that your dog may have regurgitated an intestinal worm and it just so happened to come up with the medication. Usually heartguard will kill intestinal worms as well so that is a possibility. Unless (I know it is gross) Giulio ate the regurgitated medicine again you will need to repeat the heartworm dose.

I am sorry to hear about your concern for Poppie! Depending on when your groomer saw the worms, it is possible they could have been worms killed by the dewormer and then pooped out. There are also different kinds of worms one may see in the stool, some of which are not killed by the commonly used dewormer or the dewormer in heartgaurd (such as tapeworms). I would recommend dropping a stool sample off for your vet to check under the microcsope, or if your groomer sees another worm having them save it and bring it in for your vet to check. Your vet can then prescribe the appropriate dewormer for Poppie.

Be sure that the deworming medication that you purchased is appropriate for the type of worms that are present. If Diamond's worms resemble spaghetti, they are likely roundworms, whereas if they resemble grains of rice, they are likely tapeworms. Tapeworms can be a bit trickier to treat and will likely require prescription based dewormers from your vet. As fair warning, if Diamond has a large burden of worms, she may vomit or have diarrhea with worms present, but this is usually short-lived. I hope that all goes well!

Heartworms reside in the blood vessels around the heart and lungs. You may be seeing roundworms in Alvin's anus, which can happen when a dog is being treated for intestinal worms. He needs at least two deworming treatments given two weeks apart to kill all the worms. If you don't think it's intestinal worms, then I would have him seen by a vet to diagnose what this white string is so he can be treated.
