Puppy fell off couch, hit head. What are signs of internal injury?

The first question to answer is, if he lost his consciousness when he fell. Being 'knocked out' suggest brain trauma, concussion at least. If he was conscious at all times, then most likely the brain did not suffer any serious injury. Dogs have relatively small brains (compared to human), which are also well protected by the skull bones. Brain injuries are much less common in dogs than in human patients. So, my advice is : as long as Milo is looking normal, he is bright and alert, he is not vomiting, his pupils are of equal size, he is not in any obvious pain or discomfort you should monitor him at home. If he becomes lethargic, weak on legs, crying for no reason or vomiting you need to call your regular vets or your nearest emergency service asap.

I would suggest to monitor him closely for the next 48 hours and record any abnormal sign you may notice. I would especially concern (so that would require an immediate check up) if you notice signs like lethargy, inappetence, vomiting or abnormal behaviour. Poor Winston! Given that he's walking fine, eating, and playing, I wouldn't rush him into a vet right away. There is a chance he could have suffering from some internal injuries from the fall, and I would monitor him for now. If he starts limping, stops eating, vomits, becomes very lethargic, or have any other changes in behavior from the normal, I would have him seen by a vet immediately.

Poor Louie! Keep a very close eye on him. If he appears dizzy or disorientated, has trouble walking, blood from the nose or ears, is painful, or is very lethargic, you must take him into a vet immediately.

If he’s acting normal I wouldn’t be worried. The most I would be worried about is a broken bone but it sounds like that didn’t happen. I think Rudy will be fine. Give him lots of kisses for me!!

If your dog had internal damage he would have breathing problems, or an extended hard abdomen, or blood coming from his anus or other body orifices, or would get very pale mucous membranes, get lethargic over time. If any of this happens, take Pitru to a veterinarian right away.

Luckily puppies are very flexible and rarely get injured like adult dogs. Oliver may be sore tomorrow and if there is a problem it should show up by then.
