Is Piroxicam safe for my dog? How to give it with Pepcid and food?

Anything is possible but I do not think the diarrhea is related to the Piroxicam. If you are worried I suggest you contact your vet since they are most familiar with her case and medical history. Accidents happen! Fortunately, the carprofen won't impact her heart disease or interact with the pimobendan. However, there is a chance that she could experience some stomach upset with vomiting and/or diarrhea, particularly since this is a pretty big dose for a dog her size. With this kind of dose, the worst case I'd expect to see could be ulceration of the GI tract. Normally, carprofen is prescribed at a dose of 4.4 mg/kg over 24 hours. Today, your dog consumed 7.86 mg/kg. In most cases, we don't see toxicity impacting the kidneys until a dose of 40 mg/kg is ingested. With all of that in mind, your dog should be absolutely fine! In order to be extra cautious, I'd recommend feeding her a bland diet of boiled white meat chicken and white rice for a few days, then gradually mix it in with her normal food over 5-7 days. You can also call your vet to see if they can give a dose for an antacid, like Pepcid. (It's illegal for me to do so without having examined your dog.) If you see any vomiting, diarrhea, lethargy, or blood in the stool, you should call your vet right away. I hope that everything goes well!

I understand your concerns - you can definitely read some scary things on the internet! All medications have the possibility of causing problems or side effects - thankfully, for the most of drugs that we use commonly, these issues are extremely rare. Rimadyl is a non-steroidal anti-inflammatory drug (NSAID), very similar to Advil in humans. All NSAIDs have a theoretical risk of causing side effects like GI upset, irritation, and stomach ulcers, as well as liver or kidney issues at very high doses. But just like it would be extremely rare for Advil to cause these problems in humans, it's also extremely rare to see any serious side effects with Rimadyl. This is one of the most commonly used drugs in the veterinary field, and is well-tolerated by the vast majority of dogs who take it. Personally (knock on wood!) I have never had a patient have any serious problems with it, and we use it every single day in our clinic. So while nothing is every 100% guaranteed to not cause any problems, Rimadyl is a very safe and effective drug - just make sure to always give it according to your vet's directions, to minimize the chance of any issues :)

Hello. There are no reported problems administering these 2 medications together, although there is a warning about administering Sildenafil with a similar medication cimetidine ( Pepcid is famotidine ). So just to be sure, I would separate these 2 medications by at least 30 minutes, ideally would be 1-2 hours. Since famotidine can alter the ph of the gastric mucosa & thus potentially affect the absorption of other medications, I would give the sildenafil first. There are no drug interactions between omeprazole, carafate and carprofen however, don't give them all together. You would want to give the carprofen either 1 hour prior or 1 hour after giving the others. Carafate and Omeprazole given at the same time as carprofen will inhibit it's absorption. Hope this helps. I would give her next dose of gabapentin with food at 5pm. I would not recommend giving any more Rimadyl until you can touch base with your vet tomorrow, because you're right - the vomiting could be from the Rimadyl. Your vet may be able to suggest a different medication to try if they think the Rimadyl didn't sit well with her. In the long term, there is a thought the chronic famotidine use may decrease some nutrient absorption and lead to excessive gastric acid production with possible esophageal acid reflux if it is discontinued. However, most internal medicine specialists still preferentially recommend famotidine over other antacids (such as omeprazole/Prilosec) for chronic use as it is a bit more "mild" in its mechanism of action. I know you mentioned changing foods, but consider discussing a diet trial with a hydrolyzed "hypoallergenic" diet with Brody's vet. If he has a degree of a food allergy or inflammatory bowel disease causing his vomiting and skipping of meals, these hydrolyzed diets fed for 2-3 months can remove a lot of antigenic stimulation in the GI tract and potentially prevent a lot of GI upset.
