My old dog's bloodwork is bad for surgery. Is fluid therapy safe?

I always recommend lab work. If there is a heart murmur then I recommend either an echo or at minimum chest x-rays. IV fluid therapy, and usually propofol anesthesia induction (but several induction medications are available for geriatric dogs). Of course close monitoring of vitals, an efficient procedure and keeping them nice and warm. We do a lot of geriatric procedures at our practice and very few complication rates. I recommend a veterinary exam to check how infected the tooth is. If they listen to her heart and it is fine and her blood work is stable they may be able to do light sedation only and remove the infected tooth. Bad teeth are very painful and also the dental infection can worsen the liver problem so extracting it if it can be done safely would be my recommendation. I recommend to discuss your concerns with your veterinarian. In general, pets should have their heart rate, their breathing rate, temperature and their blood pressure monitored at all times. Oscar should also have blood work ran before the procedure and make sure he has IV fluids as well. These are the best things you can provide to help ensure a safe procedure (though keep in mind there is always some risk, just like humans). Most vets today will use a "pre-med" (combination of a pain medication and a sedative) which is often a mix of an opioid and a sedative (often hydromorphone and acepromazine, or some mix of this sort), then an "induction agent" which is usually either propofol, alfaxan, ketamine/diazepam, or ketamine/midazolam, and then will use a gaseous inhalent which is pretty much always isoflurane. Any of these drugs are fine, and often the drug that is best is the drug that your vet is most comfortable using. Thiopental is an 'old school drug' which is less favoured compared to these newer drugs, however it can be used completely safely as well. Doing pre-anaesthetic bloodwork will reduce the risks involved by making sure there is no organ dysfunction prior to surgery. I would suggest just asking your vet how the procedure works, and would check about if IV fluids will be used, what monitoring is present, as the monitoring is probably the most important part of a safe anesthetic, more-so than the type of drug that is used. It really depends on the exact blood work that is being done. If it is just a routine CBC and Chemistry profile then he probably doesn't need to be fasted. If the blood-work is being done the same day as the procedure then he should be fasted because of the procedure. You should probably call your vet for specific instructions. I am sure it is worrisome to put your dog under anesthesia but the benefits of cleaning the teeth and extracting the diseased teeth outweighs the risks of the anesthesia. As long as he is healthy, the risk is minimal.

If we are talking about a basic, elective, uncomplicated surgery, many times no additional fluid therapy is required post-operatively. In non-elective procedures or procedures that were performed on sick animals, sometimes several days of fluids are required. There is no simple answer to your question unfortunately as a result.

Those lab tests look great! Yes, in some cases acute renal failure can be completely reversible with IV fluids and supportive care. This usually happens within a few days to a few weeks, so yes - this time frame would certainly fit. I'm very happy to see that things have improved so much for Tahnee! :)
