Why does my neutered dog have excessive smegma? Is it normal?

Definitely don't cut it! It could be smegma mixed with dirt. Try soaking him in some warm water and wiping gently with a washcloth. If it doesn't come off, take a new picture and make a post so we can see what it looks like after the soaking.

There is a normal yellow/green mucoid discharge (called smegma) that male dogs have to coat the penis inside the prepuce. There could also be an infection, like a bladder infection, but typically they have signs of increased thirst, frequent urination, and producing small amounts of urine at a time. To be sure, I recommend taking him to a vet for an exam and they can check the urine. If he isn't showing the other signs, I am suspicious this is the normal discharge male dogs have. Unfortunately smegma is a normal thing for male dogs. Some times there can be an infection or a foreign body in the prepuce (the space between the skin and the penis) that will cause an increase in the discharge. I would recommend that Rudy get checked out by a vet to see if this may be the cause. The vet can also discuss the morning vomiting. Some dogs have bilious vomiting syndrome. There is no specific treatment for this, but some people find that a small meal in the late evening or middle of the night, feeding a meal first thing upon waking in the morning, or antacids will help prevent the vomiting.

A certain amount of discharge, especially in intact male dogs, may be normal. This discharge is known as smegma. However, excessive licking in this area can also be a sign of urinary tract infection, prepucial infection, inflammation or allergies. Even neutered dogs can have their penis come out from time to time. If it stays out for an extended period of time or the dog cannot get it back into the prepuce this can be an emergency medical condition. It's highly unusual for a dog's sheath to need to be cleaned, especially a neutered dog. I would be concerned that Rippy may have an infection that is only partially resolving with the use of antibiotics - so perhaps the infection isn't fully susceptible to the antibiotics that are being used, or perhaps there is a foreign object that is wedged up in the sheath that is causing an intermittent infection. He likely needs to be sedated and the area fully examined, and it's unlikely that you're doing to be able to do this at home. Another consideration should be that has a urinary problem, such as a bladder infection or stones, that is making his penis irritated, and that's why he's licking it.

Sometimes the discharge comes out of the prepuce. The production of this substance is decreased when pets are neutered. You can continue to wipe it off it noted but no treatment is needed.
