My pug is vomiting, lethargic, not eating, tests normal. What now?

That duration of inappetance is very concerning. Did you family vet prescribe some therapy, anti nausea medication? It sounds like your kid needs to see a specialist. For a case like this an internal medicine specialist would be the best person to see. A full abdominal ultrasound with an experienced ultrasonographer (either a radiologist or internist) is likely the first step if blood work and xrays are normal. Unfortunately some pets cases are complex and some disease processes require more advanced testing then what family vets can provide. Likely your family vet has ruled out some common things but an internist will look for the uncommon things. Unfortunately decreased appetite is a very non specific symptom because in animals when they do not feel good for any reason they can stop eating. It sounds like their treatment didn’t resolve the symptoms. Things like pancreatitis or GI obstruction won’t show up on routine blood work, so I would recommend calling the vet today for what needs to happens next. If an obstruction is one of the vet’s concerns, they may recommend x-rays or ultrasound. If pancreatitis is a concern, then usually they need to go on IV fluids for a day or two.

Acute pancreatitis and gastrointestinal inflammation/infections are still possible as well as foreign body ingestion with material not visible on X rays (ultrasound in these cases might be more sensitive). Given the lack of improvement, I would suggest to have her checked again and have a test for pancreatitis (it is a blood test called cPLI) performed. If not eating and probably in a sort of abdominal discomfort, nutritional support and pain relief meds are needed.

I'm sorry Andro isn't doing well! Without seeing him in person, I am very limited in what I can do for him. I suggest giving him the medication, and if he keeps throwing up and refusing to eat, take him back into a vet immediately for further tests such as an x-rays or ultrasound.

I am very sorry to hear that Puffy is experiencing these problems, I am sure it is very unsettling for you both. There are a number of potential causes of such clinical signs, including dietary indiscretion, pancreatitis, forgein body obstruction, parasite infestation, infection, and ingestion of a toxin or poison. Because you have only noticed these signs over the course of a few hours I would continue to monitor Puffy at home. Rest Puffy's gastrointestinal tract by holding back food for a few hours. Then you can continue with a bland and easily digested food of the boiled chicken breast and white rice diet and provide plenty of fresh water. If Puffy does not improve, has nonstop vomiting or diarrhoea, you notice fresh blood, Puffy seems lethargic, in pain or depressed I would advise a trip to the veterinarian immediately. I am very sorry to hear about your puppy, it sounds like he is getting progressively worse so he definitely needs some additional testing/diagnostics. I cannot tell from your question whether he had x-rays or they just recommended them, but he definitely needs some x-rays and bloodwork at this point, and potentially an ultrasound of the abdomen depending on their findings. Other considerations may include pancreatitis, foreign body in gi tract (partial or complete obstruction), parasites, dietary indiscretion causing gastroenteritis, intussuception, and a few others. All of these would require further testing or evaluation to identify. Because your dog is so small it can easily become hypoglycemic if not eating. As a result, I would recommend seeing your vet or an ER vet if you cannot get in by the morning if symptoms havent inproved significantly. You can rub a little corn syrup or maple syrup on the gums to try and stave off low sugar temporarily. If vomiting becomes more frequent overnight, you shouldn't wait until morning.
