Our dog injured his paw. Do torn pieces on The carpal pad require stitches? What is the tube-like piece protruding from between the two torn pad pieces? At top of picture, what is the horizontal crease/cut? Can we treat all at home or is vet care required?

IT looks to be the pad, the underlying muscle might need stitching as it looks deep, but the pad usually cannot be stitched and a bandage that is changed every 48 hours will need to be applied for about a months.
