My dog suddenly pees everywhere, vet tests clear. What's wrong?

Your veterinarians have done an excellent work-up to rule out any underlying conditions or issues so far, but have they been able to rule out the possibility that Cali has ectopic ureters or something else congenital like urethral hypoplasia or just plain congenital incontinence? Medically, you might discuss the use of Incurin ( estriol ) either by itself or added to the Proin. If there is no response to either/both of these together, there is some literature that indicates that GnRH ( gonadotropin releasing hormone ) may be effective. If Cali remains medically unresponsive, then there is a surgical procedure to implant a hydraulic urethral occluder. Your veterinarian will probably not be able to do this, but if you are near a veterinary teaching hospital, that would be a good place to have this done. Your veterinarian can research this for you. Urinary incontinence is a common problem in older spayed female dogs. Your veterinarian can prescribe medication to control this. I’m glad you’ve checked her urine and since it was negative then starting her on medication for incontinence is the next step. I usually start with Proin, and they can add another medication if that isn’t effective. These are prescription medications so you’ll need to talk with your veterinarian about trying one. That's a tough case. If the ultrasound did not include evaluation of the kidneys and ureters (tubes leading from kidney to bladder), it may be worthwhile to have this evaluated using ultrasound or CT. Some patients have ureters that are implanted abnormally in the bladder wall leading to urinary accidents. The second question would be whether or not he has crystals in the urine which may lead to irritation and accidents. The third question is the thickening in the bladder wall...what is it? Can it be aspirated? You need to discuss these questions with your veterinarian or possibly a veterinary internal medicine specialist.

In my experience with ectopic ureter, it would be recommended to sort that out first ASAP, before having her spayed. there 2 diagnostic procedure that can be done, the first one is an xray after injection contrast media into the bladder, the second one is endoscopy. the first test can be done by your vet if he has some experience with it, the second test must be done by a specialist.

Although not very common in male dogs, urinary tract infections can still occur in males. Primary urinary incontinence can occur in males, although more common in spayed females. I would recommend having a veterinarian examine Stanley and test his urine to see if there is any evidence of a bacterial infection. If the urine is normal, then I would recommend having his blood tested to see if there are any abnormalities. If blood results are within the normal ranges, I would then recommend doing a trial of a urinary incontinence medication. I hope this helps guide you in the right direction!
