Dog's skin yeast infection returned and looks worse. Is this normal?

It is impossible to diagnose a skin condition from photos alone. Her vet may recommend doing cytology, fungal culture and/or skin scrapes to determine the cause of her infection. In some cases referral to a veterinary dermatologist may be recommended for difficult cases. Sometimes there is underlying endocrine disease or allergies that may require bloodwork to diagnose. In the meantime, a diet for sensitive skin is always a good idea for dogs with recurrent skin infection. I really like Purina ProPlan Salmon for dogs with skin problems. Many yeast and or fungal infections can take a while to improve. Veterinarians often start with topical therapy and often that is all that is needed. However, if the area is larger, not improving, bleeding, or otherwise seems worse than sometimes oral anti fungal medications and sometimes a cone to keep them from licking are important nest steps. If you stopped the Fluconazole course before you were supposed to, then that would be why the yeast infection spread from his paw to his leg. It's never recommended to stop a course of treatment unless told to do so by your vet. I would take Bruno back into your vet today for an exam, the vet can advise if they want you to continue on the Fluconazole. Itching in dogs can be causing by fleas, mites, or allergies. If he isn't up to date on his flea treatment, you will need to give him a spot on treatment such as Advantage or Revolution, and you will have to disinfect the environment. Should he be up to date on his flea treatment, I would have the vet examine and test him for mites (skin scrapings) and allergies (blood tests) when you see him for the yeast infection.

It does look like a skin infection but the origin (bacterial, fungal, yeast) I don't know just by looking at it. If it is bacterial, sometimes oral antibiotics are needed if topical therapy is not working. I don't know what cream you are using as I am not aware of a chlorhexidine cream, or how long you have been using it, so it's hard for me to comment much more on what you are already doing. I personally like these chlorhexidine wipes because they also contain an anti-fungal/anti-yeast component (climabazole) and a component to help repair the skin (phytosphingosine) so it is gentle. So, if she is licking, you will need to get an E-collar (cone) for her to wear until this is healed. So, if you want to try the medicated pads, I would do that. If there is still not resolution after a few weeks then she should go back to the vet. Skin maladies typically take quite a long time to respond completely to treatment so as long as it's not bothering her too much just continue on for a few more weeks. Fungal infections that are not responding to topical treatment often require a course of oral medications. Skin is the largest organ in the body, so it takes the longest to respond to infection treatments. Many skin infections require a full 6-8 weeks of treatment. Skin infections can also be complicated by allergies, underlying endocrine disease, or associated immune mediated or dermatological disease. A yeast infection is a good thought and the medicated shampoo would help that condition, but he may need stronger treatment to completely eliminate it. However, the yeast infection is not often the primary cause of the skin problem. More likely, it's due to an internal problem with the dog. It can also be caused by bathing too frequently with innapropriate products like human shampoo or soaps. Hypothyroidism is a condition that is a possible cause. If Kado is chunky, but doesn't eat that much, then that would raise my suspicion. This would have to be diagnosed and treated by a veterinarian. Either way, Kado likely needs a vet and it would be worth having an exam done to see what the possible causes are and the expense. In the meantime I would keep up the medicated dog shampoo baths once every other day. You should also add a dog conditioner to the routine after the medicated bath.
