My dog has a severe rash & scabs despite Apoquel. Is it allergies?

Pet's info: Dog | Mixed Breed Large (61lb +) | Female | spayed | 11 months and 4 days old | 100 lbs

Millie has a severe looking rash on her underside. A previous vet thinks it's allergies but it was never this severe. It also looks like she has a few scabs up her hind legs. She has been on Apoquel for a few months but just had this flare up. Is this more than allergies? The previous diagnosis was based on a five minute vet visit where they didn't even look at her underside (Millie wouldn't let them get close before getting agitated).

I can see a lot of redness and bumps on Millie's skin, which can be indicative of a skin infection (bacterial or fungal) or skin mites. Unfortunately, dogs with skin allergies can easily develop infections. The reason for this is that with the chronic scratching, the natural barrier of the skin is compromised, allowing bacteria to enter. Millie may need antibiotics or a medicated shampoo to clear up the infection, so she does need to see a vet. I recommend showing your vet these photos, especially if Millie doesn't like being examined. You can also try an omega-3 supplement for her along with Apoquel to keep her skin healthy.
