My dog has a red rash and firm lump on her lower belly. What's this?

That is certainly in the area where the inguinal lymph node lies. The red area there looks like a possible tick bite dermatitis. I would recommend starting with antihistamines and let your vet take a look at this area. If your pet also has a fever, then a course of antibiotics should be considered in case there is an infectious component here.
