My dog's neuter incision hurts after 4 days. Is this normal?

Based on the photo, it does not appear to be infected or overly swollen. There is likely some discomfort from the incision healing (itchiness) and pain from the procedure. If you believe that the pain is not well controlled, I would reach out to your veterinarian to discuss additional pain medications. It sounds like Siete may actually be showing signs of pain if he is shaking. If you are seeing discharge at the incision site it would be a good idea to have a veterinarian check the area out to ensure it isn't infected. The vet can prescribe pain meds or antibiotics if necessary. An Elizabethan collar (e-collar) should be used to prevent licking. Hope he's feeling better soon!

It doesn't look a 100%, i would expect it to look a little better after a week but no need to panic either, it doesn't look infected and it will heal eventually, it just may take a little longer than usual.

Although there is some bruising around the area this is quite normal and the wound looks to be clean, sealed and dry at this stage. Continue to monitor for any gaping or discharge from the wound but this is quite unlikely at this stage

Both incisions do look red, and the incision from his undescended testicle looks as though it may be partially open. Unfortunately even though it had been two weeks it looks to me like the incisions may not have been fully healed and he was able to open them by licking. An infection would also be possible. I would recommend seeing a veterinarian. They might recommend re-suturing the incisions or potentially starting antibiotics and putting the e-collar back on.

When an adult dog is neutered, there is a very high rate of swelling in the scrotum unless the dog is kept very quiet. If he is just too energetic, you may need to request a sedative from his veterinarian. The incision site has mild swelling, but appears to be healing. Be sure to keep him from licking. If it is only dripping a few drops of blood and your dog does not show any other symptoms, i would not worry about it, it can happen and it can last 3-4 days max. the blood tends to gather in the empty testicular sac and this is why it tends to bleed for several days.
