Dog's WBC count dropped after tumor surgery. Is it bone marrow cancer?

Hello, sorry to hear about Theo. This is really dependent on the type of infection. If the infection is responding well to antibiotics and her bone marrow is now starting to show evidence that it may be working, then you could likely hold off on obtaining a bone marrow biopsy after consulting it with your veterinarian. Alternatively, if the bone marrow is not responding adequately, or if an underlying cause for such a low WBC count is not able to be determined, then a bone marrow biopsy is likely the next best diagnostic step. In looking at the bloodwork you attached, I would not be concerned about the total White Blood Cell value being slightly lower than normal. After the total WBC value, the bloodwork breaks it down into individual white blood cells, like neutrophils and lymphocytes, none of which are too low. In fact, neutrophils, the body's first responders to infection or inflammation, are a little higher than the normal range. White blood cells are the fighters of the immune system, and it appears that Mary still has plenty in case she needs them. There is nothing you can do raise them other than keep her in good health. However, I do see that both BUN and Creatinine are also elevated. This can happen for many reasons, including severe dehydration, kidney insufficiency or failure, kidney infection, among others. These are the more concerning numbers that I see, and I am sure your vet has discussed a plan of action for these values.

With a white count that keeps getting higher despite antibiotics I would be concerned there is cancer hiding somewhere. An ultrasound would be my next test as it can look at the intestine, lymph nodes, liver and spleen. The spleen makes the red blood cells, and if it has a tumor then that can cause that count to drop. An immune disease can also cause a low red count.

I would make sure that all possible infectious causes are ruled out such as tick borne diseases. A chronic infection can cause low RBCs and low WBCs as well. Bone marrow could still be diseased and be leading to the low RBC and WBC count and a bone marrow biopsy in this case would prove to be useful. I hope this helps! Hi, thanks for using Petco Pet Education Center, formerly Petcoach! Why was the blood work done, is Harlow sick? If she is sick can you repost with her symptoms. Also it would be helpful to see the blood report as reference ranges are different. If Harlow just had routine blood work done and is normal then I wouldn't worry about the results and I would recheck the CBC in 1 month if she is still acting normal. Have a good day!

I can see the results, but I'm unable to see the reference ranges for each value which can vary based on the machine so it's a little difficult to say for certain what these changes could mean. It looks to me like the monocytes and lymphocytes (specific types of white blood cells) are a little low - this could be a stress response which is very normal for a dog having it's blood drawn in the hospital. Some dogs simply lie on the lower end of the reference range as well. I can't think of any disease where low lymphocytes and monocytes in the absence of other changes would be significant, and this would not raise a flag for possible cancer in my mind. Red cell distribution width (RDW) gives an idea of the average size of a red blood cell and mean cell hemoglobin concentration (MCH) tells how much hemoglobin is present in each cell - these parameters are really only significant when there is also anemia or low hematocrit. If you have other concerns you should definitely follow up with your vet - it's always easiest for the doctor who has examined the animal and is familiar with the case to interpret lab results!

If there is no detectable cancer, then the best thing to do here is treat it like an inflammatory condition with steroids +/- antibiotics through your vet. I would also consider an abdominal ultrasound just to be thorough. Elevated WBC's either means infection or inflammation in most cases, especially if cancer is ruled out.
