Pet's info: Dog | Shih Tzu | Female | unspayed | 8 months and 26 days old | 11 lbs
I have a problem with my shihtzu dog. Unspayed. 9 months old. She's lethargic , less appetite, lucks her genitals often, always follows me around. I've been looking up to some articles regarding menstruation for dogs. Most articles mention that dogs are active but why is it my dog isnt? I was thinking to go back to the doctor because I am worrying very much

Yes, this is typical of a dog in her first heat. If it goes on longer than 5-7 days, then I would take her to the vet for an exam to make sure there is nothing else going on. You can make her food more enticing by adding a little broth or small amount of boiled chicken breast. She should go back to normal as soon as the heat cycle is over.
