My dog has IBD. Are there tests before starting a special diet?

With a normal stool sample and blood panel, the next steps are experimenting with diet - specifically a hypoallergenic prescription diet (such as Royal Canin Ultamino, which is definitely the most expensive option, but also the one that seems to work best in diagnosing food allergies. It takes eating ONLY the prescription food for about a month to see if it helps, and then if it does help you can certainly experiment with less expensive foods. If you she would having signs of food allergy you'd need to do a full 3 months, but for intestinal issues, you should see a response in just a couple weeks) OR a high fiber/easily digestible diet (such as Hills Fiber Response EN). Some dogs need continual probiotics to keep normal stool. Treating any underlying anxiety (if present) can also help. Finally, a gut biopsy, which is much more invasive and costly can be done. Since she isn't yet spayed, I would consider doing a gut biopsy when/if she is spayed because that would be minimal additional expense or trauma for her while she's already have an abdominal surgery. I'm sorry to hear of your concerns with Snowball. Inflammatory bowel disease can be a frustrating problem to deal with. The only way to definitively diagnose it is via intestinal biopsies. Food trials with hypoallergenic diets can be helpful in cases of inflammatory bowel disease, but do not always solve the overall problem. Sometimes a course of steroids, supplementation a certain vitamins and even antibiotics are needed to get things back on track. Given that it sounds like Snowball has a sensitive stomach, I would not recommend feeding lettuce as it is difficult to digest. Oftentimes feeding a low-fat, easily digested diet can be helpful. If the diarrhea continues I would recommend having Snowball seen by your veterinarian. There are certain blood tests that can be beneficial to determine if any additional vitamins are needed and they can help you to develop a plan for further diagnostics or management of symptoms in a long term.

Hello, and thank you for your question. I'm sorry to hear that Chi has been unwell. If IBD is suspected, we should always try out a limited ingredient hypoallergenic diet (such as rabbit, venison, or kangaroo), for a minimum of 8 weeks with NO other treats or table scraps. However, since Chi's appetite is not the greatest, it will be a challenge to find a hypoallergenic diet that she likes. It sounds like the medications that were prescribed are appropriate for the suspected illness. Carafate is a gastroprotectant that is used to minimize gastric ulcers - it should not worsen diarrhea symptoms. Two other suggestions which I would discuss with your vet: 1) try a different type of anti-diarrhea antibiotic, such as tylosin. 2) consider weekly treatments of vitamin B12, as many dogs with IBD are B12-deficient. 3) try again with steroid therapy such as prednisolone, or try a different immunosuppresant such as cyclosporine. Most cases of inflammatory bowel disease (IBD) require hypoallergenic diets composed of hydrolyzed proteins (proteins broken down to the point where they will not stimulate the immune system). These diets are prescription based and include Royal Canin Ultimano, Royal Canin HP, Science Diet Z/D and Purina HA. In some cases of IBD, a dog can tolerate a novel protein diet (one protein source and one carbohydrate source) after a period of time on a hypoallergenic diet. Some novel protein diets are prescription based (such as Science Diet D/D), but others are available over the counter (such as Dick Van Patten's Natural Balance Limited Ingredient Diet). Most cases of IBD also require some form of corticosteroids for management in addition to the diet change. Before making any changes in Peaches' diet, be sure to consult her vet to make sure this is appropriate for her case. I hope this helps!

For IBD, immunosuppression is typically the treatment of choice in severe cases. Azathioprine is an immunosuppressant drug that is commonly used along with changing the diet to something such as Royal Canin Ultamino. I would discuss this with your veterinarian for starting something such as Azathioprine in place of steroids to see if this helps more. I would also recommend adding in a probiotic such as Purina Fortiflora, which can be purchased online or at your local pet store quite easily.

It sounds like it may inflammatory bowel disease but he would need biopsies of his GI tract to diagnose. You could try a hypoallergenic diet like science diet z/d ultra and talk to your vet about low dose steroids (budesonide is a good choice for GI issues) if you haven't tried this already. Also I would have him dewormed with a broad spectrum dewormer if you haven't already. I have had good success with this regimen in some of my IBD patients. It sounds like y'all have ruled out everything else. I hope this helps! if this doesn't work then I think intestinal biopsies are the next step.
