My dog has IBD and bloody stools. What non-steroidal treatments work?

If IBD is suspected, we should always try out a limited ingredient hypoallergenic diet (such as rabbit, venison, or kangaroo), for a minimum of 8 weeks with NO other treats or table scraps. However, since Chi's appetite is not the greatest, it will be a challenge to find a hypoallergenic diet that she likes. It sounds like the medications that were prescribed are appropriate for the suspected illness. Carafate is a gastroprotectant that is used to minimize gastric ulcers - it should not worsen diarrhea symptoms. Two other suggestions which I would discuss with your vet: 1) try a different type of anti-diarrhea antibiotic, such as tylosin. 2) consider weekly treatments of vitamin B12, as many dogs with IBD are B12-deficient. 3) try again with steroid therapy such as prednisolone, or try a different immunosuppresant such as cyclosporine. Hi there and thank you for using Petco Pet Education Center, formerly Petcoach to address your concern. With IBD, I would also recommend using B12 supplementation such as Cobalequin. Cobalequin is available online and is easy to get. This is basically a B12 supplement and is extremely bioavailable even in the presence of GI disease such as IBD. A different diet such as a hydrolyzed protein diet (a prescription diet) may also be helpful. You will need a prescription for a hydrolyzed diet and your veterinarian can provide this for you. I would try these things and see if they help at all with the soft stool / diarrhea / poor appetite. It's not clear from the history what sort of duration we're talking about, but it can take time (sometimes weeks) to get IBD symptoms under control. Many cats improve much faster if put on a tapering dose of steroids (often prednisolone is used), so you could talk to your vet about this. Depending on what diagnostics were performed that led to the IBD diagnosis, those symptoms could indicate IBD, but could also be consistent with a food sensitivity/allergy, or GI lymphoma, among other things. An abdominal ultrasound helps with a diagnosis, if that hasn't been done, with a GI biopsy being the ideal diagnostic (although that's also quite invasive, and your vet can help you decide if it's worth the risk). At this point, I'd recommend communicating with your vet and letting them know how Port's doing, as they may have been hoping his symptoms would improve with the medications you mentioned - but they may want to change things (such as adding a steroid, or discussing the next diagnostic step) once they know he's still having diarrhea. The problem with doing steroids is that it decreases the inflammation in the intestines and will likely make Cosita feel better, but if her disease is due to cancer (lymphoma), putting her on prednisone will make it harder to diagnose and treat her cancer later. "Lymphoma is temporarily responsive to prednisone but better responses can be obtained from stronger chemotherapy agents. Exposure to prednisone will make the lymphoma much more difficult to diagnose should biopsies be obtained later. Plus, exposure to prednisone can lead to resistance to other medications. (This is less of a problem for cats, but in dogs even a few days of prednisone can make a lasting remission impossible to achieve.)" There is nothing wrong with starting her on steroids, but you have to understand that it will limit your treatment and diagnostic options in the future if the steroids do not help. Good luck! Many dogs with IBS will respond to a food trial or hypoallergenic diet. You may want to discuss prescription diets with your vet. Thanks for using Petco Pet Education Center, formerly Petcoach! He is adorable! I think that a course of prednisone could potentially be added to better control the IBD so it has an all day effect. Another thing that can be tried if your veterinarian (who knows all his medical history) thinks is a good idea is changing him to another IBD type diet such as Royal canin ultamino. Aw I am so sorry to hear this about her. Her illness is likely what is causing the decrease in appetite. The steroid usually helps to decrease the inflammation and increase the appetite. A few things I would talk to your vet about is whether a daily probiotic and a long term appetite stimulant would help her. I would also talk to your vet about starting her on weekly Vitamin B12 injections. Most dogs with GI cancer or IBD have low Vitamin B12. Your vet should be able to give her one injection a week for 4-6 weeks then decrease the injections to once a month if she is doing well. Vitamin B12 is also a natural appetite stimulant. I would also talk to them about whether an oral steroid daily would be better than the occasional steroid injection. I obviously do not know what type of steroid injection they are giving. Some steroid injections stay in the system for 6-8 weeks where others only stay in the system for 48 hours. I would also consider putting her on a hypoallergenic diet if she would be willing to eat it. If she prefers human food, you may consider making her own diet at home.
