Gg had a heat spot which the vet shaved and prescribed lsaderm cream for. But it kept spreading and now after I shaved it a little more and I think she scratched it again when I left the room to get her stuff and now it looks like this. is it safe to keep using this cream? I'm freaking out how much blood is coming out this. Was never this bad the last time she got it.

It looks as if your dog has a terrible hot spot. This is a local site of severe infection and inflammation. He will likely need antibiotics and a steroid injection to get it under control. This can be a painful condition and needs to be treated as soon as possible.
I hope this helps. Thanks for using Petco Pet Education Center, formerly Petcoach. Best of luck.
