My dog has high creatinine, won't eat. Is she in pain? Options?

Pet's info: Dog | Pomeranian | Female | unspayed | 10 years old | 25 lbs

My pet has 116 mg/dl of BUN and 5.9 mg/dl of creatinine . We restricted her protein diet and trying to give her ice cream and potato, rice but she is not taking anything from past 1 day. Its a very tough situation for us as we read that if creatinine levels are above 5, that means 80-90% of kidneys have been destroyed. What is best way: to keep her alive/on fluids or to end its life peacefully? We love her soo much but we can't see her dying with alot of pain.Can we knw how much pain she is in?

Based on your post of Lucy's bloodwork she has both elevated liver and kidney values which could mean she has another underlying condition altogether that is affecting both her organs such as leptospirosis, Cushings disease (hyperadrenocorticism), or any number of tick diseases. Therefore she needs additional testing to determine what the primary disease process is.
She needs to most likely be hospitalized, placed on fluids to flush her system and pain medication if indicated. while the vet runs the necessary tests to determine the underlying problem here so that we know how to best treat her. Like I said before these lab values cannot be differentiated unless additional testing is done. I would likely first start with finding the specific gravity of her urine to rule out kidney failure first. Just because elevated kidney enzymes are present it does not always automatically mean that the pet is in kidney failure. Also it is very unlikely that both the kidneys and liver enzymes will be elevated without having another underlying disease process causing this. I hope that your pet Lucy is able to make a full and speedy recovery. Take care and best of luck.

    Poor Lucy. With a BUN/Cr that high their prognosis is definitely poor for a good recovery although I have seen it. If you can afford it that I would recommend hospitalization with an IV catheter and fluids for a few days. They can also give her injections of anti-nausea medicine. If this isn't an option then I would recommend to give her subcutaneous fluids (under the skin, prescribed by your vet) daily alone with 10mg Pepcid twice a day. You can also ask your vet to see if they carry the oral medicine cerenia which is great for nausea. Finally talk to your vet about an appetite stimulant as this may help too. I find they eat best once you can get the BUN under 80.

    Now what is the best choice? That is a hard decision to make. Just know the kidney disease itself isn't painful but it makes them feel really crummy. If she isn't responding well to treatment after a few days or a week then I would consider euthanasia as she will likely not respond much after that. You can have your vet check her blood work again to see how she is responding.

Hello. it looks a lot like an acute toxicity, probably something she ingested as the liver enzymes are very high as well. i don't know how long it has been like this, but the longer these levels remain the less chances of survival. i believe she needs to be on fluid therapy in the hospital 24/7 and hope for the best.

It sounds like Lucy is in pain, poor girl! Yelping, lack of appetite, and decreased movement are all signs of pain in dogs. It's very possible she sprained or strained something, like her back or a limb, and this is her way of telling you. I'm glad the Cosequin seemed to help her, but in general, Cosequin is not a pain reliever; it's a joint supplement that provides nutritive support to cartilage, joint capsules, and joint fluid, and it's an excellent product, so I do recommend you keep Lucy on this consistently. However, I think she needs some more aggressive pain management and a check-up with your vet to make sure she hasn't seriously injured herself. Especially if her back is painful, as that can sometimes involve pinched nerves. If she has a history of bladder stones and is having inappropriate urination, that definitely needs to be evaluated as well. When you go in to have her pain assessed, bring a urine sample with you for your vet to check. It may be something as simple as a urinary tract infection or it could be a recurrence of bladder stones, but it's worth getting checked out. Good luck and I hope Lucy feels better soon!

Poor Bubbles! There are so many things that can cause these things. An elevated creatinine level could be secondary to true kidney issues or dehydration or other illnesses. I would have your vet do a urinalysis on her to see how well her urine is concentrated. If it isn't directly related to the kidneys I would be looking for other things that can affect the kidneys like dehydration, leptospirosis, addison's disease, etc. I would also have your vet check her fecal for any parasites. Your vet should also be able to do x-rays to ensure there isn't a blockage or tumor in her abdomen. I hope this helps!

Thank you for submitting your question regarding Linda. I would stop feeding any treats and keep her strictly on the kidney diet. She should have her blood pressure checked and a urinalysis done to look for extra protein in the urine. Both are common with kidney disease. There are medications that she can take that will help both conditions if necessary. This will slow further progression of her disease. Depending on her results, I will add a potassium supplement (if her potassium is low) or a phosphate binder (if her phosphorus is high). These things will help her to feel good for as long as possible. Always ensure she drinks plenty of water. I hope this information helps!

My cat has kidney and liver disease. What can I do to help her?
I'm so sorry Lucy has been diagnosed with kidney and liver disease. Unfortunately I would need to know a little more about her particular case in order to give more specific advice on treatment and prognosis. Acute kidney injury can be caused by toxins or infections, and with aggressive IV fluid therapy and hospitalization sometimes the kidneys can recover full function. Chronic kidney disease happens more slowly, and while diet changes and monitoring for secondary problems like high blood pressure can help slow the progression of disease there is no cure. ALT and cholesterol can increase for a number of different reasons - gastrointestinal infections, pancreatic disease, and liver and gallbladder infections would be a few. It seems like her kidney disease is likely more significant at this point. If you have more specific questions you can consider starting a consultation with an expert or repost your question with a little more detail about her history and current treatments.

Oscar's lab work is pointing to both liver and kidney issues. Unfortunately it's impossible to tell if this is truly a kidney problem or just dehydration secondary to other issues (not eating, not drinking, losing fluids through vomiting and diarrhea) without knowing the specific gravity of his urine. Possible causes these values could include liver diseases like viral or bacterial hepatitis, cancer of one or more abdominal organs (liver and kidney), or the bacterial disease leptospirosis. Unfortunately it's impossible to say for sure how to treat Oscar without more testing, and based on just these values alone. At the very least IV fluids may help to treat either dehydration or kidney disease, so you can talk to your veterinarian about starting there, but ultimately there are more questions to be answered.

My dog has very high creatinine and kidney damage. What can I do?
I expect Crazy is very ill with these values. If this value just happened due to a toxic episode or infection aggressively treating with intravenous fluids and medications for several days may prevent permanent kidney damage. That would require hospitalization. If this is a chronic problem that has been progressing the outcome is not good. You can give fluids and a prescription kidney diet but in my experience these dogs unfortunately do not get better. I hope you can treat Crazy and that he responds.
