My dog has heartworm. Can he get doxycycline after his first shot?

A "low positive" may mean a low adult heartworm burden. The preventive is to kill any immature heartworm (microfilariae). The doxycyline is to kill the wolbachia bacteria. This is an organism that lives within the heartworm parasite as a symbiont. This portion of the treatment lasts 30 days after which medication is given via injection to kill adult heartworm. Both doxycycline and prednisone can cause vomiting and diarrhea. I am not sure of the dose that your dog is on but maybe your vet could adjust the dose so he has less gastrointestinal problems. Feeding the medication with a meal can sometimes help with the gastrointestinal side effects so make sure to give with food. Your vet could also prescribe an anti-vomiting medication, probiotic, and GI protectant which may help, too. At this point, If he is just going to vomit the medication than it doesn't make sense to give it. You should wait and see if your vet can prescribe additional medications to help with the vomiting and diarrhea. If the vomiting and diarrhea has stopped, you could try to give one medication at a time with food and see how he does.

I am so sorry that Tucker has tested heartworm positive. I adopted a dog that was heartworm positive, so I certainly understand how difficult it is to make a decision on treatment. I am attaching a link below to the American Heartworm Society (AHS) which has a lot of good information for you. https://www.heartwormsociety.org/ The AHS does not recommend the "slow kill" method as it's treatment of choice, but does recommend the following for an alternative treatment if needed. They recommend Doxycycline and either ivermectin or topical Moxidextin. ("Topical moxidectin is now FDA-approved for use in heartworm-positive dogs.") They do not list ivermectin or topical Moxidextin as being better than the other, so they should have the same efficacy. Injectible Moxidectin (Proheart) is not listed by the AHS as recommended and the company that manufactures Proheart lists that it "should be used with caution in heartworm positive dogs". I am attaching the exact recommendation from the AHS below. " In cases where arsenical therapy is not possible or is contraindicated, the use of a monthly heartworm preventive along with doxycycline at 10 mg/kg BID for a 4-week period might be considered. An antigen test should be performed every 6 months and the dog not considered cleared until two consecutive NAD (no antigen detected) heartworm antigen tests, 6 months apart, have been obtained. If the dog is still antigen positive after one year, repeat the doxycycline therapy. Exercise should be rigidly restricted for the duration of the treatment process." The recommended dose of doxycycline for Lyme disease is 20mg/lb once daily, so Joey should be taking around 200mg a day. Rarely doxycycline is used at higher doses or more frequently than once every 24hrs. Please, get in touch with your local vet to discuss the treatment as it sounds that the dose should be slightly adjusted.

Doxy is the right medication, but he definitely needs to be on a tick prevention product like nexguard or advantix every year during tick season in your area to prevent further infestation by ticks.

The Melarsomine is not effective against juveniles under 4 months of age. It is recommended to administer a heart worm preventive for 2 months prior to administering melarsomine. This will decrease new infections, eliminate existing larvae, and will let older worms (between 2 and 4 months of age) mature so they would be more susceptible to melarsomine. Hello, sorry to hear about Zoey. It truly depends on the dose your dog got. If the weights are similar then Zoey may not experience any side effects. If the weights are different, then it truly does depend on what dose in mg/kg Zoey ingested. If you think this dose is high then you could expect to see vomiting, diarrhea and a decreased appetite.
