My dog has enlarged liver, normal bloodwork, vomiting. What is it?

Pet's info: Dog | Yorkshire Terrier | Male | neutered | 10 years and 10 months old | 11 lbs

Jackson has been throwing up (mostly bile), has dark diarrhea, hasn't been eating normally, & drinking alot of water. We took him to the vet last night & they did bloodwork & an x-ray. The bloodwork came back normal, but the x-ray showed a slightly enlarged liver. We had the x-ray sent out to a radiologist & haven't heard anything yet. The vet gave us anti-nausea medication for him & we need to report if he continues to throw up. With an enlarged liver & normal bloodwork, what could this be?

Hello. It is good that the blood work was normal as this indicates there is no damage or destruction going on with the liver cells. Mild inflammation could cause the liver to swell slightly, especially since the liver is the main organ for removing toxins from the bloodstream. This means the liver is doing its job. And, this might just be an artifact of positioning during the x-ray or Jackson may just have a slightly larger liver than normal. If he seems better, then the medication is doing its job. If he continues to act unwell, the next step would be to ultrasound his liver to get a better picture as to what might be going on, & perhaps additional blood work to narrow things down. But I would not be too concerned just yet, as the liver has a great capacity to repair & regenerate itself ( the only organ in our bodies that can do so ). I hope Jackson is feeling better soon. I am sorry for the delay in answering your question... I hope that Iomy is feeling better! Based on his xray, it does appear that his liver may be a bit enlarged. If he is still showing symptoms of illness, ask your family veterinarian about an abdominal ultrasound. This may help clarify what the problem is and what your options are. Also, if blood tests haven't been done that would also be helpful. Mild increases in liver values can be seen for a number of different reasons, and sometimes it can be very difficult to determine what the underlying cause might be. Infections like leptospirosis, migrating parasites, congenital diseases like a portosystemic shunt or some toxins can cause liver values to increase. It seems like Leo is on supportive medications for the liver - it would be helpful to know if his values are increasing or decreasing since trends are most important. An abdominal ultrasound might be the next step, or you could also ask about referral to a specialist if available in your area.

My dog's blood work is elevated, but he shows no illness. Why?

There are many different causes that would include hepatitis (which can be bacterial or sterile), cancer, or an issue with his gallgladder. The next step I would recommend is an abdominal ultrasound with fine needle aspirate (FNA) to try to get some information about what is going on in the liver, and possibly a biopsy of the liver after that. The other courses of action is to try treating him to various medications to see if anything helps -- however this doesn't really make sense if he has no signs of illness. Or finally, doing nothing and checking again in 2 months to see if they are rising or staying the same. Sometimes we decide not to get to the bottom of something when there are no clinical signs.

My dog has high liver enzymes and an enlarged liver. Should I worry?

Elevated liver enzymes and abnormal liver appearance on ultrasound scan does not necessary mean liver tumour or liver failure. Older dogs often have abnormal liver appearance, which is not related to any serious underlying problem. Liver enzymes level is a measure of 'damage' to liver cells but it is not directly related to prognosis. Again, older dogs often have mild elevation of liver enzymes I would think that the specialist will perform full abdominal scan and possibly take liver samples through a needle - this is commonly performed procedure in dogs with signs of liver disease. Based on the findings of the aspirate, the doctor will recommend further testing or prescribe treatment for Mars.

My dog is vomiting constantly and not eating after vet visit. What's next?

The bilirubin is a bit high, as is the ALP (alkaline phosphatase) but neither of these values is super specific in terms of what the cause or the best treatment is. We can see the ALP be elevated with stress, age, and sometimes even normal cell regeneration within the liver. The bilirubin can be slightly elevated when the blood sample is what we call "hemolyzed," which can sometimes happen when blood is spun down for a blood test to be run. Therefore, if he is still showing symptoms of illness, I would echo what Dr Ana said; it may be time to consider imaging. Either X-rays or an abdominal ultrasound maybe helpful in identifying the problem and therefore identifying the best way to treat it. Take him back to the vet if he's still having troubles, as he probably is in need of some more fluids as well. My cat has an enlarged liver. Is it serious? What tests are needed?

Hello and thank you for contacting Petco Pet Education Center, formerly Petcoach. Liver enlargement means that the liver is bigger than it should be. It could be because of liver inflammation due to infection (bacterial or viral) or toxins ingestion. In young cats like Arya liver tumors are very uncommon, but enlargement of one liver lobe could be caused by a liver abscess or a cyst. The best way to assess liver size is ultrasound scan and/or x-ray. Blood test tells about liver function. In other words, it tells if the liver, ,despite being enlarged or diseased, is still able to maintain its function. Blood test also can assess the extent of liver damage. It is hard to say anything about the prognosis without knowing why the liver is enlarged and what are the blood results. In general, liver have huge capacity to recover from any insult, but again it really depends what has happened.
