My dog's chronic ear yeast infections won't go away. What can I do?

Allergies can cause dogs to have chronic ear problems. You could ask your vet about trying medications for allergies and see if that helps. I would also recommend an ear culture to make sure that he is doesn't have a resistant infection. You could also switch him to a hypoallergenic diet and see if that helps.

Yeast ear infections often need antifungal topical ear medications to help resolution. Most topical ear medications will also contain an antibiotic (not always needed) and a steroid, which will help with the itch the yeast can cause. I would recommend that you discuss the proper medication with your veterinarian. Many times yeast ear infections are caused by too much moisture in the ear (like after swimming in a pool or lake) or from inflammation caused by allergies (from fleas, food, environment, etc.). If Tiki has recurrent yeast ear infections, you may need to have her vet work up allergies as the primary problem. For minor yeast ear infections that aren't red, swollen, or itchy, you can try an ear cleanser to help remove the yeast. If you do this for a couple days and it isn't better, than she will probably need prescription medications.

If it didn’t help the infection, she may have a bacteria that is resistant to the antibiotic. Poor Maggie! This looks like quite a severe ear infection, from what I can see in the photo. Normally, we would treat this by taking a sample of the discharge to look at under the microscope, to see what kind of infection is present - it may be yeast, bacteria, or both. Then appropriate medication can be prescribed to treat the infection depending on what is found. So that is how we would typically treat a current, active infection like this. If her ear infections are not responding well to treatment or keep recurring once meds are finished, then this usually indicates that the dog has an underlying allergy problem of some kind (either food, or environmental). The underlying allergy will need to be diagnosed and treated in order to get the recurring ear problems under control, so that's what I would focus on once the current infection has improved. Your veterinarian should be able to discuss testing and treatment options for allergies, including allergy testing and immunotherapy treatment, daily medication such as Apoquel or Atopica, and/or a food trial with a prescription hypoallergenic diet. You could also seek a second opinion from another veterinarian in your area, or request referral to a veterinary dermatologist, if your current vet doesn't have any concrete suggestions on how to proceed with this.

It is impossible to diagnose a skin condition from photos alone. Her vet may recommend doing cytology, fungal culture and/or skin scrapes to determine the cause of her infection. In some cases referral to a veterinary dermatologist may be recommended for difficult cases. Sometimes there is underlying endocrine disease or allergies that may require bloodwork to diagnose. I would recommend not just an ear wash but also medications to clear up this infection. If there is moderate-severe or 2+ yeast and cocci then I would recommend Oticetic flush and Mometamax ear medicine. You can also try Zymox ear medicine as an alternative. You typically can only get these through your vet or with veterinary approval. Ear medicine and medicated cleaners are typically not sold over the counter. The discharge inside the ear needs to be cultured so you can know what is growing in the ear canal - what bacteria, yeast, etc. That way the vet will know what medication to use to get it to clear up. The ear may also need to flushed out under anesthesia. There must be an underlying cause. Cats typically don't get ear infections chronically like dogs. It is not likely to be a food allergy. More testing needs to be done.
