Senior cat lymphoma: Is surgery safe, or treat presumptively?

Hello. I’m so sorry to hear about your cat. I had a lovely long haired cat with lymphoma years ago. There is lymph tissue throughout the body. Before deciding on any types of surgery, it is beneficial to know just how extensive her cancer is. If she, for example, has a section of intestinal tract with a tumor or a couple of tumors, that area can be removed. However, if it seems that the cancer is already metastasized to liver, spleen, lungs, etc. , her prognosis would change and possibly surgery would not be if much benefit. A treatment plan if surgery/ chemotherapy / both can be arrived at after more information is collected.

I'm sorry to hear that Sweetie has a stomach tumor! While that is a slightly elevated SDMA value, it is difficult to say exactly how her kidneys are doing without the remainder of her bloodwork. As long as Sweetie does not have spread of her cancer to other organs, especially the lungs, and her heart is functioning normally, anesthesia may be safe. She will also be on fluids throughout the surgery and monitored carefully. Your veterinarian can go over the risks associated with anesthesia with you considering the remainder of Sweetie's blood work. The prognosis for survival once the tumor is removed depends greatly on the type of tumor it is and how well the stomach is able to heal following surgery as well as if you elect to pursue chemotherapy. Hi, thanks for using Petco Pet Education Center, formerly Petcoach! Poor Phoebe! Lymphoma is a diffuse cancer. It usually isn't a solitary mass but is throughout the entire organ. In Phoebe's case all her lung tissue. So Lymphoma is treated with chemotherapy not surgery usually. Since you are willing to treat, ask for a lung aspirate or biopsy. They can biopsy the lung, culture it for bacteria and do a fungal culture. Sometimes you have to be the health advocate for your pet! It's not an easy surgery and aspirates are risky. But if you want to know what is going on then you should try to get an answer. Also since you have been seeing a specialist maybe they can use a scope to get the samples. You are right, there is always some risk with surgery. However, you may want to visit with your veterinarian about the anesthesia monitoring they do. It may surprise you how low the risk is and how much effort is put into safety. As far as the mass goes, I do get concerned anytime a mass is changing. I do recommend surgical removal any time there is a big change to an existing mass. However, there is always the possibility that it could remain quiet and never cause a problem. Poor Kitty! Since she is already 17 years old and she is having such a tough time with her inflammation in her GI tract, I think steroids will be the best choice for her. If her blood work looks okay now, then I think she can tolerate the steroids. Unfortunately, if you don't put her on steroids the lymphoma or IBD may continue to get worse. It is true the steroids will not CURE lymphoma, but it certainly give her a better quality of life right now. It could keep the IBD under control and eventually you may be able to decrease her steroid dose to a lower dose. The concerns of long term steroids are much lower since she is already geriatric. If she was 4 or 7 years old, then I would be much more worried with starting her on long term steroids at that time. At the age of 17, our goal is to give Kitty are good quality of life. If you want more options about the lymphoma and interested in seeing an oncologist, I would ask your vet for a referral for a oncology consult. I hope this helps!

Usually even with chemotherapy you are looking at a prognosis of 2-6 months depending on the exact type of lymphoma. You could try prednisolone as a palliative treatment to make her feel better for the time being and perhaps extend her survival time for a few weeks. If she is already anemic, she may also require a blood transfusion making things a bit more complicated. If she is not doing well and you are not interested in pursuing therapy knowing the prognosis then euthanasia is a good option. Let me know if you have further questions regarding her disease. In a senior cat, there is always a chance any lump could be cancer unfortunately. If it is cancer, the vet will likely want to do further tests to see if it's spread. There may be chance this is just a polyp, and removing it is curative. You need to be aware that any surgery carries risks due to anesthesia, but the vet wouldn't recommend the surgery if they didn't feel it was safe for him. I do recommend having the surgery done.
