Why is my pregnant cat peeing by doors? Medical or behavioral?

That's very kind of you to take in this stray cat! This peeing on the bed can be a sign of a medical illness, such as a UTI or bladder stone. I would recommend taking your girl into your vet tomorrow for an exam. Your vet can do routine blood tests and a urinalysis to see if she does have a UTI or bladder stones. If she has a UTI, she will need antibiotics to treat it. Bladder stones may need to be removed surgically, or can be managed with diet.

Urinating outside the litter box is a very common behavior problem reported to vets. Inappropriate urination can be a way of your cat letting you know they aren't well. It can signal a urinary tract infection, kidney disease, diabetes, or hyperthyroidism, among other things. The first step to treating this is to take your cat to the vet for a health check. Your vet will do a urinalysis to see if there is blood or bacteria in the urine (signaling a UTI), or glucose in the urine which can suggest diabetes. Your vet will also do routine lab tests such as a biochemistry to check organ functions. Once a medical problem is ruled out as the cause of the inappropriate urination, your vet can discuss behavioral methods with you to help stop this behavior.

If you litter box is covered, I would try uncovering it and adding an additional uncovered litter box somewhere else in the house. If your vet has already ruled out an urinary tract infection, I would also consider adding some feliway diffusers around the house. Feliway is a cat pheromone which can help to calm stressed cats and help with inappropriate urination. If this doesn't help, I would talk to your vet about starting an anti-anxiety medicine like amitryptiline. I hope this helps and he stops soon!

It sounds like your cat might be marking, which some female cats will do. However, she should be checked by a vet to make sure there isn't a medical cause for her urination, such as a urinary tract infection or stress cystitis. It maybe a urinary infection. If this goes untreated it can lead to an obstruction and acute kidney failure. I would have your vet make sure there is no medical reason such as that causing it. If all is normal it is behavioral. Just because nothing new has happened inside, does not mean something new is not happening outside. There may a new cat or other animal that is stressing yours or scaring yours and you will have to look into identifying that and removing it
