Why is my old cat sick, not eating, and peeing outside litter box?

Tommy is experiencing very general symptoms of disease, these symptoms could fit almost any internal condition, maybe he is suffering from nausea due to a gastrointestinal disease and maybe he is in pain, from the symptoms described it's very hard to know. i would recommend taking him to the vet as soon as possible in order to get him checked, if he doesn't show any other obvious symptoms during the examination, a blood test and possibly an ultrasound will be needed in order to try and find out what is wrong with Tommy.

It sounds as though Tommy may be in pain if he is unable to walk normally, refusing to eat, and hissing. Cats often isolate themselves when they are experiencing pain. It would be best to take Tommy to a vet for a physical exam and any necessary diagnostic tests to try to determine why he is experiencing these issues so that proper treatment can be started. Poor kitty! In a 13 year old cat, I strongly suspect that she has a medical problem causing these issues. Some common possible causes would include hyperthyroidism, diabetes, and chronic kidney disease. I would recommend taking her to the vet for an exam as soon as you are able to - he/she can do blood work and a urinalysis to help diagnose the problem and get her started on appropriate medication to treat it.

There are many possible causes: fever, infections, ate something that upset his tummy, or not being able to pee. You didn't say how old he is but metabolic problems like kidney disease or diabetes is also possible. If his symptoms continue then have your Vet look at him tomorrow. If he isn't urinating/straining to urinate then have him seen ASAP. I wish you the best and hope Thomas feels better soon!

I agree that it sounds like Tommy is not feeling well at all. Squinting eyes often indicate a respiratory tract infection though other issues like a lack of tear production and scratches on the cornea can also look the same way. The fact that he has little energy is very worrisome. Cats who feel bad often don't drink and eat the way they should to maintain health. They are prone to dehydration. Upper respiratory infections can also move into the lower respiratory tract and cause a pneumonia. Older cats are prone to many diseases, including kidney disease, hyperthyroidism, diabetes, infections, cancer, etc. He should be examined by a vet for a work-up with blood-work and a urinalysis to help diagnose and then determine the best way to treat and manage his problems.

Loss of appetite and lethargy can be observed with many conditions such as gastroenteritis, dietary indiscretion, kidney disease, pancreatitis, among many other possibilities. Defecating outside of the litter box is usually a medical issue and can be linked to everything else going on. I recommend that you have Merlin evaluated by your veterinarian for further assessment and treatment. They would be able to check for these possibilities and others depending on the findings on exam.
