My old cat is peeing and pooping everywhere, then lying in it. Help!

Some common possible causes would include hyperthyroidism, diabetes, and chronic kidney disease. I would recommend taking her to the vet for an exam as soon as you are able to - he/she can do blood work and a urinalysis to help diagnose the problem and get her started on appropriate medication to treat it.

The vet can prescribe medications for her, and there are a few things you can do at home. You can get Feliway, which is a plug in diffuser that releases pheromones into the home. These pheromones help cats with anxiety. You can can also get her a Sentry calming collar. This collar release pheromones as well when in contact with her skin.

But in a cat this age, I would be very suspicious of an underlying medical issue causing this behavior - this can be true even if she appears generally healthy otherwise, on the outside. Possible causes would include hyperthyroidism, a GI problem such as chronic pancreatitis or inflammatory bowel disease, or arthritis making it difficult or painful for her to get into the litter box when she needs to go. I would recommend a vet visit for a thorough physical exam and some basic lab work to check for these issues. If she has a medical problem of some kind, having it treated appropriately will often resolve the unwanted accidents.

Poor Sophie, she looks very sweet. It sounds to me like she is having serious digestive issues and needs to be examined by a veterinarian. She likely is pooping outside the litter box because she has increased urgency to go and can't hold it as long due to a medical issue. If she is drooling it's likely she may have dental disease so your veterinarian can examine her mouth and address the problem. With her balance being off I'm also concerned she may be in pain and require prescription pain medication. Please remember there are no safe over the counter pain medications for cats so you will need to have these prescribed by a veterinarian. So sorry that she is having problems and it is always a very difficult decision. Have her examined by a vet to see if there is anything that can be done to make her more conformable. She might be having hip and/or back pain making it difficult to get in the litter box. An anti-inflammatory or pain medications may help make her more comfortable and improve the quality of her life. If not, then it might be time to consider euthanasia. 20 years old is amazing and she is certainly lucky that you rescued her!
