My kitten is coughing; X-ray shows growing white spot. What is it?

I'm sorry to hear that Mittens has not been doing well. I agree that Mittens' chest x-rays appear to be progressing over the past 2 weeks. At this point, either pneumonia is less likely or the bacteria causing a possible pneumonia are resistant to her current antibiotic therapy. Cancer is absolutely a suspicion due to the x-ray progression. Consider asking your vets if they have considered performing either endotracheal or transtracheal washes on Mittens, which would be able to obtain a sample of fluid or cells in the lungs. These samples could be analyzed for cytology (which may be able to diagnose cancer) or culture (to determine if bacteria is present and an appropriate antibiotic). There is a chance that Mittens may not be stable enough to undergo sedation for this procedure, but your vets will be able to examine her to determine if the risks are worth the benefits. She doesn't need to be under anesthesia for xray, just a sedation that can be reversed in seconds, an xray is a MUST at this point, lung cancer needs to be ruled out as well as asthma, heart failure and other respiratory diseases, this cannot be done without an xray.

It is best to have your veterinarian give you their opinion based on the X-ray and the exam findings, but -in my opinion- this is not a normal appearing chest X-ray for a kitten. The white densities visible in multiple lung lobes may represent consolidation which may indicate pneumonia. This may be due to a bacterial infection or fungal infection or lung parasite. I would definitely recommend further discussion with your veterinarian to start a new course if treatment- whether that is another antibiotic or another type of medication would be based on your vet's recommendation.

Your vet seems to be thinking that Ember is suffering from a severe infection, which is one possibility but i think a couple more things need to be ruled out completely in order to be sure that she is getting the correct treatment. it could be cat asthma, although usually it will show on the xray, it might be worth having the xray sent to a radiology specialist to rule that out completely. it could also be heartworm, although less common in cats than dogs this parasitic disease could cause these symptoms. there is a blood test available that can rule this disease out as well.

There could be fluids in the lung indeed but it could also be just the upper respiratory tract that is blocked, as it's been 2 weeks the type of antibiotics probably needs changing, i would recommend switching to an antibiotic called Zithromax which is usually the best option for this type of infection.

Other possible causes of cough include heart disease, lung parasites, allergies, asthma, fungal infections, pharyngeal polyp, among many other things. Along with this, you may notice sneezing, runny eyes or runny nose. Other possibilities would include pneumonia (lung infection), hairball cough, or feline asthma (which would be rare in a young kitten). If Ash's coughing is continuing overnight, I would recommend taking him in to the vet tomorrow morning. They can perform a full physical exam, check for fever or dehydration, and prescribe the appropriate antibiotic medication if an infection is suspected. If Ash seems to be getting worse tonight, is having trouble breathing, or is lethargic, I would suggest getting him to the emergency vet tonight.
