Why is my cat's third eyelid still showing after vet treatment?

Your vet prescribed a general antibiotic eye drop which also contain steroids, from my experience when both third eyelids go up into the eye like this, it is usually not an eye related disease, it's more likely to be an intestinal disease. if she has not been dewormed lately, i would recommend starting by deworming her, often it is the discomfort related to the worms in the intestines that is causing this symptom. once the abdominal discomfort will be over, this symptom will disappear.

The elevation of the third eyelid really just tells me that the eye is uncomfortable, it doesn’t really make a diagnosis. It can be a viral infection, an injury, allergies, or others. I would warm compress the eye several times a day, as well as gently rinse with an isotonic saline rinse several times a day. This can be purchased at any pharmacy in the human eye section. From the history it appears that she had rather severe upper respiratory infection. The eyes are commonly affected with upper respiratory infections and the raised third eyelid occurs with infections, so I think that is what your vet is treating for. However, this can also occur with a condition call Horner's Syndrome where the pupil decreases in size compared with the other. I can't tell from this picture if the pupil is actually smaller (the thrid eyelid is in the way), but if it is, I would have her checked for signs of Horner's. Having the third eyelid up is often a sign of a systemic disease from a virus or infection, not an eye problem itself. This can also be caused by intestinal parasites. I recommend a veterinary exam and testing of his blood for feline leukemia and aids and a fecal test for intestinal parasites. It is not a specific sign for one particular disease but it often means that a cat has fever, has breathing problems, has abdominal discomfort, muscle or joint pain. Poor kitty. Cookie may have an eye infection, a lesion on her nerve (Horners syndrome) or an allergic reaction. However the bloody butt is something completely different and requires a complete exam to determine if this is coming form the stool or anal glands. All of these conditions will requires a vet visit I'm afraid to get to find out the cause and then prescribe medications to treat. There aren't any over the counter options that will help Cookie. Cats with really painful eyes (such as from a scratch to the cornea or something stuck in the eyelids) will elevate their third eyelids to help protect the eye. Though some types of neck injuries can damage a nerve to works in the face and can also cause this to happen. Since I am concerned that this beautiful kitty is in pain, I would recommend taking her to the emergency vet. After the examination, discuss financial constraints with the vet to determine what can be done to alleviate pain but still fit within the budget you have set.
