It could be those nipples are used more often than the others, leading to the different appearance. The size of the kittens could be due to genetics, or it could be these kittens are not getting enough milk. You can have them all checked out by a vet to make sure everything is ok. Even if she is just painful from engorgement you can get pain medications from your vet. There is unfortunately nothing you can give at home that is safe. Please have her into a vet as soon as you can, and if you are noticing pain/swelling/hot/cold then you should get her in ASAP. Good luck with her!
