My cat has recurring ear infections. What are the best treatments?

Ear mites and underlying allergic disease (that can predispose to recurrent ear infections) are possible and should be ruled out. A physical and otoscopic examination of the ear canal are enough to rule them out. Bacterial swabs and culture can be used as well to rule out resistant infections. I would recommend to discuss all these options with your veterinarian. If the antibiotics are helping, it's probably a middle ear infection indeed. a sample needs to be taken from the ear and sent for culture and sensitivity, then the best antibiotic can be chosen according to the result. the course of treatment must be longer next time, at least 4 weeks as these infections take a long time to resolve.

Poor cat. Her balance issues are very suggestive of an inner ear infection, which also means that she probably has a ruptured ear drum due to the chronic ear infections. Some pets are prone to repeat ear infections because of stenotic (narrow) ear canals, polyps inside the ear (quite common in cats, also will have nasal discharge), and a thyroid issue (not as common in cats). I would recommend getting Daisy back to the vet for oral antibiotics to treat the inner ear infection and just a sterile saline wash to clean the ears out, since no topical medication is safe to use in a ruptured ear drum. Then I would seriously consider checking for polyps and if not found then consider having a total ear ablation surgery for Daisy. This involves removing the inner ear canal which will essentially prevent any future ear infections from occurring. This is a drastic procedure and should be thoroughly considered after discussing the pros and cons with your veterinarian. But considering the severity of Daisy's ear problem, it definitely seems like an option that may be in her best interest. If it didn’t help the infection, she may have a bacteria that is resistant to the antibiotic. I recommend taking her back to the vet for a culture of the bacteria in her ear to determine what medication will treat it. Good luck and kisses to Bailey!

The discharge inside the ear needs to be cultured so you can know what is growing in the ear canal - what bacteria, yeast, etc. That way the vet will know what medication to use to get it to clear up. The ear may also need to flushed out under anesthesia. There must be an underlying cause. Cats typically don't get ear infections chronically like dogs. It is not likely to be a food allergy. More testing needs to be done.

Poor Bailey - it is vital he is seen by a vet again as soon as possible to have the condition reassessed. Ensure he is fasting as it may be necessary to sedate him in order to fully examine the ear and identify any infections or growths which may be present and to ensure a deep clean can be performed. In some cases further tests for underlying causes such as allergies or thyroid disease may be recommended and in very rare cases surgery is required to remove or bypass areas with permanent scarring which may be preventing the normal healing process. Regular, routine cleaning of ears each week is recommended for dogs with recurrent problems

Ear infections can be caused by bacterial, parasitic or fungal infection, or a combination of these, and can be associated with dietary or environmental allergies or sensitivities, immune system disorders, or a combination of the above. Your vet can look at the sample under the microscope and treat appropriately based on what is found. Trying different medications without testing to determine the cause is not effective, and can lead to bacterial resistance to medication. Your vet will likely need to examine the ear with a scope to see if the ear drum is intact, as some medications cannot be used safely if the eardrum is ruptured. They may also recommend a culture and sensitivity test of a swab from the ear to help figure out what specific bacteria could be present and what medications they are sensitive to. Ear infections can be very painful, and as you mentioned, can sometimes progress to include the middle or inner ear, where they can cause neurologic signs or permanent hearing loss.
