This is probably due to a severe allergy, although it could also be fleas, infection or mites.

Poor Girl! This could be an auto-immune disease or something called eosinophilic granuloma complex. You could try some balm called snout smoother on it to see if it makes a difference. The vet can do blood tests, skin scrapings, and use a Wood's lamp to diagnose allergies, mites, and ringworm. Once the cause of the itching is found, the vet can prescribe the appropriate treatment.
